Job Responsibilities include:

The Case manager is accountable for the organization, sequence of services, and resources that are necessary and appropriate for the achievement of patient outcomes within effective time frames on a specific group of patients. In addition, the Case manager will coordinate the plan of care among all members of the health care team.

The Case Manager must have ability to practice under minimal supervision. The Case Manager will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, protocol development and monitoring, development of plan to address physical and psychosocial needs, coordinate services and discharge planning, patient/family teaching, and monitoring of cost effective health care across the continuum of care. The Case Manager must complete all initial admission assessments within 24 hours of admission.

The Case manager must meet resource management expectations, assist to reduce practice variations, and enhance patient outcomes. Reports to the Director of Case Management, supervised by Case Management Team Leader. There are no subordinates who report directly to the Nurse Case Manager. Interacts with others both inside and outside of the organization, including, but not limited to, members of the department, supervisory staff, other ancillary staff members, physician office staff, and outside vendors.

Education:

Graduate of an accredited school of professional nursing. Prefer Certified Case Manager, CCM

Experience:

  1. 1-3 years of recent acute care experience 2) Minimum of 6 months Case management/Utilization Review experience preferred 3) Previous experience in discharge planning and managed care preferred 4) Strong written/verbal communication and computer skills 5) Organizational skills with a well-developed style of diplomacy, assertiveness and a tolerance for ambiguity 6) Knowledge of Case Management concepts; interest in the financial aspects of patient care

Licenses/Certificates:

Current licensure as a Registered Nurse in the State of Texas required. Current CPR certification or obtain within 60 days of employment.

Laredo Specialty Hospital, located in Laredo, TX, is a 40 bed facility providing long-term acute care services to patients recovering from serious illnesses or injuries. Often our patients require additional critical care services for medically complex conditions such as trauma, infectious diseases, wound healing, cardiovascular disease, stroke, amputation and ventilator weaning. The Admissions Manager is responsible for coordinating and planning the daily operations for all pre-admission activities, including coordinating admission with other departments by collecting and distributing the appropriate information. Responsible for identifying information needed from the referring hospital to finalize the admission. Leads the admission function with responsibility for the conversion of patient referrals to admissions by verification of accurate clinical and financial information and prompt communication with all stakeholders in the admission process. Works closely with the business development team and hospital leadership to exceed the hospitals revenue, census, conversion and admissions goals through excellent communication and responsive customer service. Qualifications/Skills : * Current, valid, and active license to practice as a Licensed Practical/Vocational Nurse, Registered Nurse or Respiratory Therapist in the state of employment required. * Minimum three (3) years of clinical healthcare experience required. * Knowledge of Medicare, Medicaid, and insurance procedures required. Additional Qualifications/Skills : * Bachelor's degree preferred. * Case Management/Discharge Planning experience preferred. * Supervisory experience preferred. * Knowledge of general admission procedures and basic office procedures, practices and equipment preferred. * Ability to project a professional image. * Knowledge of regulatory standards and compliance requirements. * Strong organizational, prioritizing and analytical skills. * Ability to make independent decisions when circumstances warrant. * Working knowledge of computer and software applications used in job functions. * Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
