Christiana Care Health System, a Magnet designated healthcare organization with two acute care hospitals, is seeking a Case Manager to work in the Wilmington Hospital. This is an exceptional opportunity for an experienced professional RN to work in a collaborative, team environment.
This position will be part of a case management team integrated within the core operations of a team of physicians, nurses, nurse practitioners, and other professional staff. The Case Manager will be responsible for evaluating patients and developing the CM plan of care, utilization management, coordination of all post-acute services, and post-discharge follow-up appointments.
The RN Case Manager will provide longitudinal care management for an identified population of patients/members in conjunction with the patient's primary physician provider, according to agree-upon clinical guidelines, with the objective of coordinating high quality patient care across all care delivery sites.
The Case Manager utilizes clinical expertise, discretion, and independent critical thinking skills to assess, provide, coordinate and monitor healthcare services for patients assigned to the clinical unit. The case manager will collaborate with the multidisciplinary team of physician, nurses, nurse practitioners, and other professional staff. The case manager will work with the patient and family to develop a safe and effective discharge plan that will meet the needs of the patient beyond the hospital stay.
Required Education and Experience:
Bachelor's degree in Nursing with an active Delaware license is required.
At least two years of acute care nursing experience required.
Previous Case Management experience preferred.
Certification in Case Management as a CCM is preferred.
Candidates must have excellent teamwork and assessment skills, be proficient at developing the CM plan of care, and the demonstrated ability to manage a large caseload of patients very efficiently.
