Position Summary

The April Parker Foundation is seeking a dedicated and compassionate Case Manager to join our team in our Rapid ReHousing program. As a Case Manager, you will play a vital role in identifying, screening, and assisting individuals and families with accessing social support services for rapid rehousing. Working closely with our Outreach Team and community partners, you will conduct compassionate outreach to individuals and families experiencing housing instability, providing strength-based case management, conducting assessments, and making community-based referrals and linkages.

As a Case Manager for the Rapid ReHousing program, you will be responsible for providing ongoing support, crisis intervention, and housing stability support to program participants. You will help connect individuals with rental vacancies, assist with completion of rental applications, and provide guidance and support throughout the leasing process. Your knowledge of the local housing market, including rental trends and resources, will be essential in assisting program participants in securing and maintaining stable housing.

Requirements

Minimum Qualifications (Knowledge, Skills, and Abilities)

  • Bachelor's degree in psychology, counseling, social work, or a closely related field
  • A minimum of two years of related experience OR a Master's degree in social work, psychology, counseling, or a closely related field
  • Experience with rapid rehousing programs strongly preferred
  • Knowledge of housing resources and rental market trends in the local area
  • Strong case management skills, including assessment, service planning, and referral coordination
  • Excellent communication skills, both verbal and written, with the ability to effectively communicate with program participants, community partners, and external organizations
  • Ability to work independently and collaboratively as part of a team
  • Familiarity with homeless services systems and experience working with individuals and families experiencing housing instability
  • Proficiency in using case management software or platforms

Benefits

The salary for this position ranges from $49,920 to $58,240, depending on experience.
