Job Description: Overview

We are currently seeking a full-time Case Manager for our El Cajon Comprehensive Treatment Center. Hours are 5:30a - 2p.

El Cajon CTC, a member of Acadia Healthcare's Comprehensive Treatment Centers, is a part of the leader in medication assisted treatment for individuals struggling with an opioid addiction. With full-circle care that includes a blend of therapies and the use of safe and effective medications, we are prepared to treat the entire problem, not just a piece of it.

Position Characteristics

The Case Manager is a licensed professional who manages the patient's cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to patients. Provides quality care through developing, implementing, managing and evaluating patient care plans. Collects clinical data, correlates this clinical data and provides clinical input to the treatment team. Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted. May also correlate clinical data for business office as required.

Responsibilities

  • Providing case management and care coordination services for patients in the clinic. Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;

  • May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;

  • Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team's relationship within the facility or treatment milieu; and,

  • Perform other duties as assigned.

Job Requirements

  • Bachelor's degree in Social Work, Counseling or related Human Service field from an accredited college or university

  • 1-2 years of experience working in behavioral health or addiction recovery setting preferred.

  • Previous experience in case management and/or utilization review preferred

Licenses/Certifications:

  • Current and active clinical licensure or active endorsement to license

The Case Aide will be responsible for answering multiple phone lines and gathering information from callers. The Case Aide will also be responsible for gathering information, data entry and providing appropriate referrals and/or necessary services as needed to in-house residents as well as callers.<br />  </p> <p>The ideal candidate will be ethical, courteous, respectful and compassionate with good teamwork skills, help clients achieve wellness and autonomy, and go the extra mile to ensure patient progress and satisfaction. The Case Manager should have strong knowledge of HIPAA, client confidentiality, and demonstrate good attention to detail.<br />  </p> <p>The Hawley Veterans Center is a transitional housing program for homeless & disabled veterans located in El Cajon, CA.<br /> <br />  </p> <p><strong>Essential Duties & Responsibilities:</strong><br />  </p> <ul> <li>Adheres to Volunteers of America and Program/Facility Policy and Procedures.</li> <li>Maintains safety, security and cleanliness standards as outlined by Volunteers of America's policies and procedures.</li> <li>Makes hourly rounds of program and document client attendance on roster appropriately to ensure compliance to "House Rules", Program Policies and Procedures, and Safety Program requirements.</li> <li>Takes attendance of clients at curfew and complete warning and violation notices as needed.</li> <li>Supervises client chores and complete warning and violation notices as needed.</li> <li>Performs office upkeep duties as assigned, including the daily cleaning of the Office areas, includes dusting, filing, sweeping. Ensures client activity areas are neat, clean and orderly and all activities, books and craft material are visually presentable for external inspection.</li> <li>Prepares and serves meals and snacks according to menu, and follows-up with the cleaning of the kitchen and dining room areas to include washing dishes and emptying trash.</li> <li>Reports and records any unusual incidents immediately regarding resident behavior, filling out incident reports when necessary.</li> <li>Midnight shift: Case Aide will be responsible for preparation of sandwiches for following day's resident lunches and preparing and serving breakfast for all residents and cleaning kitchen and dining room</li> <li>Answers, screens and documents all incoming telephone calls.</li> <li>Makes appropriate referrals, conducts initial screenings for eligibility, and schedules intake appointments as appropriate and assigned by Case Manager.</li> <li>Files all papers, documents, reports in alphabetical or numerical order as requested.</li> <li>Maintains all client records in an orderly and neat manner.</li> <li>Update clients progress records promptly as assigned.</li> <li>Make photocopies as required. Complete client files for initial intake into facility as assigned.</li> <li>Assists coworkers as necessary.</li> <li>Documents client observations/incidents in logbook daily.</li> <li>Assists with house meetings, resident activities/groups as needed.</li> <li>Assists with transportation of residents, as needed.</li> <li>Assists with facility laundry as needed.</li> <li>Assists with the deep cleaning of kitchen appliances (to include, but not limited to the refrigerator, stove, oven, freezers, and vent).</li> <li>Performs all other duties as assigned by the Case Manager.</li> </ul> <p><strong>Required Credentials, Abilities, & Experience:</strong><br />  </p> <ul> <li>Experience working with homeless, mentally ill, and/or veterans preferred.</li> <li>Minimum of a High School Diploma or equivalent required.</li> <li>Veteran strongly preferred</li> <li>Must acknowledge and adhere to all Volunteers of America's policies and procedures.</li> <li>Must have ability to assess conditions and situations of individuals requesting emergency services.</li> <li>Must demonstrate the ability to communicate effectively both orally and in writing.</li> <li>Must demonstrate the ability to interact effectively with all residents, family members and monitoring representatives.</li> <li>Must have the ability to comprehend and implement instructions.</li> <li>Must have the ability to make quick and sound judgments in time of crisis.</li> <li>Must have the ability to recognize and evaluate a resident's need for medical attention.</li> <li>Must have ability to display proper telephone etiquette.</li> <li>Must have knowledge of standard office procedures and equipment.</li> <li>Must meet the eligibility requirements needed to obtain 1st Aide & CPR certifications and Food Handlers Card within 60 days of employment.</li> <li>Must posses a valid California Driver's License and insurance.</li> <li>Must obtain TB/Physical (w/in 7 Days) of employment.</li> </ul> <p>EOE M/V/D/F</p> <p>This Employer participates in E-Verify.</p> <p> </p> Volunteers Of America Southwest El Cajon CA
