ESSENTIAL FUNCTIONS:

Provide case management and care coordination services for clients/patients/residents in the facility.

Responsible for coordinating continuum of care activities for assigned clients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements.

Responsible for psychosocial assessment and for conducting individual, family and group sessions.

Conduct various duties related to coordinating treatment and discharge activities, as needed.

Provide quality care through developing, implementing, managing and evaluating client/patient care plans.

Collect, correlate and provide clinical data to the treatment team.

Convey medical criteria and clinical information between the insurance provider and treatment team as warranted.

May also correlate clinical data for business office as required.

Coordinate in a timely manner, issues or activities relevant to the treatment team.

OTHER FUNCTIONS:

Perform other functions and tasks as assigned.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

Master's degree in Social Work, Counseling or related human service field required. One or more years of experience working in behavioral health setting.

Experience with the acute mental health patients preferred. Previous experience in case management and/or utilization review preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

Current Texas license required: LMSW, LCSW, LMFT, LPCI, or LPC. CPR and CPI certification required (training available upon hire and offered by facility).

WORK SCHEDULE:

PRN has no guaranteed hours. An ideal candidate would need to have availability in the evenings and weekends primarily, with some availability during the day time Monday - Friday.

The SHS is a hub of excellence for forensic mental health and complex psychiatric care, with all facilities accredited by The Joint Commission. The hospitals provide state-of-the-art treatment that is recovery-oriented and science-based. Whether in a direct-care or support role, your contribution will make a difference in the life of a Texan. The Employee Health Nurse/Clinical Case Manager is directly supervised by the Assistant Chief Nurse Executive and serves as a liaison to outside medical facilities to ensure continuity of medical care services, by physically seeing each patients at those facilities on a daily basis. He/she serves as a consultant to clinical nurses and other program staff regarding medical/surgical needs relating to patient assessment, recovery planning, intervention, and evaluation, from admission to discharge. Conducts audits to ensure changes in patient condition were appropriately identified and addressed, looking at the 72 hours prior to emergency transfer, and documents findings in the electronic medical record. Works with counterpart on the Vernon/Wichita Falls campus to see hospitalized patients from the alternate campus when applicable. In addition to patient care services, is responsible for the promotion of employee health and well-being. Provides all new employee with education, and any necessary immunizations such as the Hepatitis B series, Td/Tdap, influenza and completes a tuberculin screening test. Initiates an employee file for each new employee and sends reminders for subsequent Hep B injections, annual flu vaccine/declination, and annual TST screening. Maintains a data base for the purpose of tracking all employee information to be reported to the Infection Preventionist, as required by the Agency. Serves as an active member of the Awards and Wellness committee; organizes an annual on-site health fair. In addition, coordinates six blood drives per year to promote community support. Works to secure services to employees at local gyms and exercise facilities. Takes the lead in the promotion of Agency wellness initiatives such as weight loss promotions. Coordinates employee services with General Medical Services and Risk Management when staff have been injured or have had an on the job exposure, and initiates paperwork as necessary. Works with the lab to set up appointments for initial and subsequent lab studies. Maintains open communication with the Infection Preventionist as it relates to such exposures. Submits quarterly infection control reports to the Infection Preventionist. Participates in General Medical Services morning report when there is a patient receiving services at an outside medical facility in order to promote continuity of care via a verbal report, and to prepare for the individuals discharge needs. Serves as investigating officer for Formal Death Reviews; organizing the information and prepares any necessary reports. Assists the Nurse Educator with competency practicum for new nurses performing tuberculin skin testing. Provides cross departmental services for General Medical Services LVN when the need arises. . Performs routine nursing work. Works under moderate supervision, with limited latitude for the use of initiative and independent judgement. Supervise General Medical Services LVN on their respective campus. Employee has access to limited set of records in accordance with HIPAA Regulation, access level of Health Care Operations PHI 2-B. Essential Job Functions: Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. (20)% Measurement: * Reviews all 24-hour reports to ensure awareness of any emergency patient transfers. * Participates in General Medical Services morning report when there is a patient receiving services at an outside medical facility in order to promote continuity of care via a verbal report, and to prepare for the individuals discharge needs. * Personally visits each individual, on a daily basis, when admitted to an outside medical facility. * Provides detailed information regarding the patient's status and plan of care. * Provides consultant to clinical nurses and other program staff regarding medical/surgical needs relating to patient assessment, recovery planning, intervention, and evaluation, from admission to discharge. * Conducts audits to ensure changes in patient condition were appropriately identified and addressed, looking at the 72 hours prior to emergency transfer, and documents findings in the electronic medical record. * Works collaboratively with counterpart to coordinate medical/surgical and employee health services in order to minimize expenditure of resources and optimize simplification of implementation. (10%) Measurement: * Completes, receives, reviews, and files employee injury reports when there is an on the job injury. * Performs a systematic assessment of the injury and documents findings. * Reviews employee's file to ensure all relevant vaccinations are current. * Works collaboratively with the Risk Management Department to provide consultation as it relates to employees who have been injured on the job, in order to decrease workman compensation costs. Description: Coordinates employee services with General Medical Services and Risk Management when staff have had an occupational exposure on the job. (10%) Measurement: * Follows up on all occupational exposures; providing education and support and initiates supportive documentation according to HHSC, CDC, and hospital policies and procedures. * Maintains open communication with the Infection Preventionist as it relates to such an exposure. * Coordinates with the lab to arrange appointments for initial and subsequent lab studies. * Submits quarterly infection control reports to the Infection Preventionist. Description: Maintains a work area that is free from clutter and survey ready at all times. (10%) Measurement: * Documents and files are organized and up to date. * Mothers room is clean and uncluttered as well as the refrigerator with the temperature maintained as per policy. * Personal items are minimal and only present when absolutely necessary. * Temperature in the medication refrigerator is maintained as per policy; is clean with no food items stored with medication. All drugs are labeled and stored as per policy. * Any personal refrigerator is clean with items labeled as per policy. * Items stored under the sink, above the shelves etc. are according to joint commission standards. Description: Serves as investigating officer for Formal Death Reviews; organizing the information and preparing all necessary reports. (5%) Measurement: * Prepares all relevant documents for the administrative and clinical death reviews including a detailed summary and recommendations to the committee of any identified administrative and/or clinical concerns and possible improvement options. * Information is prepared in a timely manner, is organized and available for committee members consideration. * Demonstrates in depth knowledge of the case and is able to field questions. Description: Assists the Nurse Educator with competency development for tuberculin skin testing during new nurse orientation.
