Landmark Hospital of Salt Lake City has an exciting opportunity for a part-time Case Manager to join our dedicated and professional team. Landmark Hospitals are committed to providing the highest level of customer service and quality of care to those we serve.
Shift: Day (20 hours per week)
Responsible for coordination of the patient's plan of care and the resources utilized for the patient's stay. Scope of the case management process involves assessment, planning, negotiating, coordinating, monitoring, evaluating options and services, and reporting to meet the health needs of the individual patient.
Coordinates patient care and facilitates discharge within the expected length of stay
Ensures that specific patient outcomes are achieved and that variances are evaluated and addressed in collaboration with members of the healthcare team
Assists in the development, implementation and revision of critical paths, and clinical outcomes data collection
Ensures that the patients' unique needs are met in the most appropriate, timely, cost-effective manner possible
Coordinates and oversees the utilization review committee and performs concurrent utilization review as defined in the utilization review plan
Effectively communicates utilization review findings with team assist in facilitating an appropriate continued stay or discharge
Supervises and coaches the case management/discharge planning team
Strictly follows isolation and infection control procedures
Registered Nurse license in State of Utah
Minimum 1-2 years case management experience, preferably in hospital environment
Excellent communication and interpersonal skills
Superior analytical and critical thinking skills, team, detail and goal oriented
Case Management Certification including CCM, ACM or NACCM preferred
Positive, professional and empathetic demeanor
LTAC/Acute Care Hospital experience strongly preferred.
