EMPLOYMENT RELATIONSHIP: Just cause subject to probationary period

GENERAL SUMMARY:

Under direction, provides direct services to consumers, family members and designated others. Implements program activities. Participates in research and program evaluation. Provides statistics as requested by administration. Links, monitors and coordinates consumer services. Coordinates care with physical health care provider(s). Develops a plan of service using a person-centered planning process.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Conducts assessments, service planning, specialized intensive case management, living skills, training, crisis intervention, advocacy, linking and monitoring for assigned consumers.

Performs home and community-based interventions, including transportation of consumers.

Maintains department productivity and quality measures.

Complies with MCCMH documentation requirements using electronic medical record (EMR).

Completes required MCCMH training and meets continuing education requirements as determined by the professional licensing board.

Maintains professional relationships with the provider community and consumers.

Coordinates with family, aftercare programs, and ancillary support services to secure aftercare.

Monitors consumer services to evaluate effectiveness through face to face consumer contact, facility visits, consultation with family members/significant others and/or service providers and records review.

Measures the effectiveness of interventions to determine outcomes and identify needs of the consumer.

Required to participate in on-call rotation as directed.

Operates an automobile to perform assigned job functions.

Performs related duties as assigned.

QUALIFICATIONS:

Required Education and Experience

  • A Bachelor's Degree in Psychology, Social Work, Special Education, Counseling or other mental health related field from an accredited college or university

  • One (1) or more years of professional experience working in case management or supports coordination with persons who have mental illness or intellectual/developmental disabilities

  • Must meet qualifications as specified in the MDHHS Michigan PIHP/CMHSP Qualifications per Medicaid Services as a QMHP (Qualified Mental Health Professional):

    1. Individual with specialized training or one year of experience working with a person who has mental illness; and is a psychologist, physician, educator (with a degree in education from an accredited program), social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, therapeutic recreation specialist, licensed or limited licensed professional counselor licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, or a licensed physician's assistant or


    1. An individual with at least a bachelor's degree in a human services field

Preferred Education and Experience

  • A Master's Degree in Psychology, Social Work, Special Education, Counseling or other mental health related field from an accredited college or university
  • A minimum of one (1) year experience working in a hospital behavioral health setting

Required Licenses or Certifications

  • Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines

  • Must possess and maintain a current Michigan Chauffeur's license within two (2) weeks from date of hire
