POSITION SUMMARY:
The Case Manager is a Licensed Nurse or professional in a health care-related field; responsible for
program development for specialty subacute care unit programs in designated hospitals or facilities through education to the community including healthcare providers and managed care organizations. The Case Manager conducts on-site and off-site physical evaluations or collects clinical data of potential subacute candidates for admission to the specialty program.
RESPONSIBLE TO:
The Case Manager is responsible to the District Directors
POSITION RELATIONSHIP:
Will be a liaison to Administration, Subacute Medical Director, Director of Nursing, Clinical Manager, Social Worker, nursing staff, business office, U.R., and admitting staff of host facility. Will coordinate with discharge planners, social workers, physician(s) of area facilities and insurance case managers. Professional presentation and maintains a professional relationship with all coworkers, healthcare professionals and clients, acts as a "Company representative" to our client facility.
QUALIFICATIONS:
RNs
Graduate of an accredited school
Additional education/work experience in the area of program development preferred
Current California License
Five years of progressive nursing in acute care and rehabilitation, Subacute or home health
Experience
Prefer Case Management experience
Broad knowledge of nursing principles, procedures and practice
General knowledge of Infection Control standards and practice
Member and participant in professional nursing organizations
LVNs
Graduate of an accredited school
Additional education/work experience in the area of program development preferred
Current California License
Five years of progressive experience in the healthcare arenas of acute care, rehabilitation, Subacute or home health experience
Prefer Case Management experience
General knowledge of infection control standards and practice
