The Case Manager assumes a critical role within the healthcare ecosystem, partnering closely with physicians and fellow team members to orchestrate the seamless transition of patients from hospital to home. With a focus on proactive, complex, and safe discharge planning, they collaborate intensively to assess, strategize, implement, and assess discharge plans, ensuring optimal patient outcomes.
Qualifications
Job Qualifications:
Bachelor's degree of Social Work required.
Basic Life Support (BLS) through American Red Cross or American Heart Association required.

Responsibilities
Job Responsibilities:
Collaborate with the interdisciplinary team and patient/appropriate decision maker to develop an individualized discharge plan.
Demonstrate assessment skills to collaboratively identify post-acute care needs.
Communicate discharge plan to all healthcare team members, including the patient/appropriate decision maker and representatives from involved external agencies.
Include patient/appropriate decision maker and interdisciplinary team in evaluating progress and modifying discharge plan to achieve desired outcomes.
Develop and maintain positive relationships with all internal and external customers.
