Summary of position

The Case Manager works in collaboration with the multidisciplinary team to establish a plan of care across the system for North Shore patients and residents. Case Manager may be responsible for arranging home health, home medical equipment, home infusion, skilled nursing facility placement, long term acute care, hospice, and out-patient rehab services. The role may include explaining benefits, assessing eligibility, completing part or all the admission process and providing daily inpatient visit services. This role partners with the business development team to build and maintain referral source relationships.

Qualifications:

  • Education: Graduate in Nursing or Social Work from an accredited program. Bachelor's or master's degree preferred

  • Licenses/Certification: Nursing or Social Worker license in the State of Wisconsin required.

  • Experience: Minimum of two years of experience in case management, home health care, hospice services or a post-acute care setting. Demonstrates knowledge of the referral source types and community resources. MDS experience preferred but not required.

Essential Functions:

  • Performs onsite visits/reviews in the hospital to determine clinical needs and appropriate post-acute care facility placement.

  • Follows patients from acute care setting to post-acute care setting and assists interdisciplinary team with discharge planning.

  • Coordinates transitions with other service providers as needed (i.e., specialty care, home health, palliative care, hospice).

  • Identifies opportunities to assist/refer patients in coping with diagnosis and potential outcomes including advanced directives, palliative care and end-of-life concerns.

  • Assist in completion and finalization of discharge planning assessments, level of care determination on admission, benefits coverage evaluation and any other required clinical assessments.

  • Attends interdisciplinary team meetings for discharge planning and clinical care coordination needs.

  • Follow up with patients after discharge home to ensure their accommodations are met and has direct oversight of the customer service outcomes.

  • Performs other duties as assigned.

Knowledge/Skills/Abilities

  • Understands Nursing Home Regulations, medications, payer sources, Therapy/Clinical Requirements for short-term rehab and long-term care.

  • Knowledge of facilities clinical capabilities

  • Understands and can navigate hospital referral sources.

  • Ability to organize and prioritize.

  • Ability to maintain confidentiality.

  • Strong organizational skills.

  • Must have proven skills in working independently, self-motivated and goal oriented.

  • Ability to communicate effectively, both orally and in written form.

  • Ability to establish and maintain working relationships with multiple and varied people and organizations.

  • Demonstrate self-confidence.

  • Ability to work flexible hours, as job duties and responsibilities may dictate.

  • Must be capable of maintaining regular attendance.

  • Must be capable of performing the essential job functions of this job, with or without reasonable accommodations.

Physical Demands

  • Ability to sit, stand and walk for varied lengths of time.

  • Ability to lift up to 30 pounds unassisted.
