Duties and Responsibilities:

Case Manager

Case Managers facilitate continuity and quality of patient careduring hospitalization. These experienced clinicians helpachieve optimal clinical outcomes by coordinating the efforts ofan interdisciplinary team and establish a safe and appropriatedischarge plan to meet an individual's specific healthcareneeds.

Position requires a thorough understanding of theprinciples and practices of clinical management of care throughthe use of critical pathways, nursing care, quality assurance,utilization management and discharge planning principles.

Preferred Criteria

  • 2 years' of case management/care coordinationexperience in an inpatient, HMO, insurance, or other outpatientsetting

  • 1 year of leadership experience in a healthcaresetting

  • Master's Degree in healthcare related field
