You will collaborate with the individual, family, physician, and interdisciplinary teams to coordinate healthcare services to promote optimal clinical, financial, and satisfaction outcomes. Responsibilities Provides counseling and crisis intervention to families and patients. Conducts psychosocial assessments. Serves as a resource to patients and families. Facilitates/ Assists patients and families in open discussions regarding advanced care planning. Being a patient advocate to ensure their needs are met and rights are protected.
