Case Manager

Milford Regional Medical Center is a full-service, community and regional teaching hospital located in Milford, Mass. and serves patients from more than 20 towns in Central Massachusetts. Statement of Purpose:

In collaboration with other members of the healthcare team, the Case Manager is accountable for coordinating and facilitating the management of care for an assigned group of patients.# The Case Manager#ensures#the#provision of a well-coordinated individualized care experience through the delivery of outcome oriented patient care within a fiscally responsible framework.# This is done#with an emphasis on early assessment of discharge needs, resources available, and communication among healthcare team members and family.# These services are maintained along a continuum and encompass pre and post hospital needs. Qualifications: Education: BSN required. # Certification / License:

Current Massachusetts RN licensure. CCM a plus.# Experience/Skills: *# Three to five years of clinical experience demonstrating the ability to make sound judgments with knowledge of current medical care technology and ancillary supports.

*# Previous UR/QA or community experience.# # *# Assertive; excellent interpersonal skills necessary to interact effectively with all levels of professionals within the hospital and in the community. # *# Analytical ability to interpret patient-related information to evaluate appropriateness of continued stay and/or need for ancillary services.
