Kindred Healthcare is seeking a Case Manager for Mercy Iowa City Rehabilitation Hospital, our new, 40-bed Inpatient Rehabilitation Facility (IRF) located in Coralville, IA! Our new rehabilitation hospital in partnership with Mercy Iowa City is scheduled to open in April 2020 and will offer specialized programs for people who have experienced stroke, brain and spinal-cord injuries, amputations, complex orthopedic injuries and other conditions requiring inpatient rehabilitative services. Summary:

Coordinates management of care for a specified patient population; follows patients throughout the continuum of care and ensures optimum utilization of resources, service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration of functions of case management, utilization review and management and discharge planning.

Education:

  • Appropriate minimum degree for preferred licensure or certification or commensurate experience in a related healthcare field.

Licenses/Certification:

  • Healthcare professional licensure preferred as Registered Nurse, LPN, Respiratory Therapist, Physical Therapist, Occupational Therapist or Social Worker.

  • Appropriate certification in Case Management preferred; for example, Commission for Case Manager Certification (CCMC); Association of Rehabilitation Nurses (ARN) certification.

Experience:

  • Three years clinical experience. Prefer experience in Case Management, Quality Management, Utilization Review, or discharge planning.

Job Summary: Responsible for coding ICD-10 Inpatient Rehabilitation admissions and discharges. Assisting in planning, developing and maintaining the Health Information Services department of the hospital in accordance with state and federal guidelines, accreditation standards as well as hospital policies and procedures. Responsibilities: * Codes IRF records using current ICD coding version and within timeframe set by hospital. * Works closely with the PPS Coordinator in assigning the Impairment Code for all admissions. * Transmits the IRF PAI in a timely manner for all Medicare inpatients. * Maintain confidentiality of all patient care information to ensure patient rights are protected. * Assists in planning, organizing, directing and maintaining Health Information Services in accordance with established policies, procedures, state and federal regulations and accreditation standards (i.e. HIPAA, TJC, CARF). * Assist in establishing and maintaining procedures for the collection, coding, abstracting, filing and retrieval of medical records. * Work with staff physicians in getting records completed within the required timeframe. * Guide clinical staff in keeping records neat and orderly during the patient's hospitalization. * At discharge, process records within required timeframes: assembly, analysis, coding, and abstracting, IRF PAI Transmission. * Review and process requests for patient information within required timeframe. * Develop and maintain good rapport with inter-disciplinary team members to help ensure that medical records are properly maintained and problems areas identified and corrected in a timely manner. * Maintain a neat and orderly filing system and overall work area. * Performs other related duties and activities as directed and assigned. * Participates in quality assessment, auditing, and improvement program activities. * Participates in continuing education classes and training programs. * Ensure supplies have been replenished in work areas as necessary. * Other duties as assigned. Experience/Skills * RHIT (or eligible) or RHIA (or eligible) or Coding Certification by nationally recognized organization.
