THE ROLE: LCS Intervention's Team continues to grow! We are adding more full-time positions to our State funded contracts. We are more than just a "job", we are a team of people who care about serving our community, partnering with other agencies, and supporting one another to be as successful as we can be. The ideal candidate will want to be part of a team, while also working independently, have experience (either working or lived) parents who battle addictions, mental health, homelessness, disabilities, DV, etc. Fathers, parent mentors, foster parents, bi-lingual, are all encouraged to apply. This individual will provide services to families referred through DHS/Self Sufficiency to enhance family stability and support workforce readiness. Provides a comprehensive screening to determine immediate needs. Provides skills training to parents and adolescents to address immediate needs. Develops and implements an effective service plan. Collaborates with community partners and connects families to relevant resources to support progress.

ESSENTIAL JOB FUNCTIONS/MAJOR JOB RESPONSIBILITIES:

  • Provide in-home change services to parents, children, and adolescents

  • Assess for parental capacities as related to child safety and well-being

  • Help family members overcome barriers and identify strengths

  • Report any potentially dangerous situations or safety plan violations

  • Use Motivational Interviewing, Collaborative Problem Solving, Wraparound, and other effective practices

  • Help parents improve diminished protective capacities

  • Knowledge of dynamics of high-risk families including substance use and behavioral health

  • Coordinate case activities with Contract Supervisor, DHS/Self-Sufficiency, schools and other community partners

  • Maintain client confidentiality

  • Provide a range of services including parent education, communication skills, home management, budgeting, stress reduction, stabilization of home environment, and other appropriate services

  • Maintain accurate and current client records, including weekly progress notes, treatment goals, monthly progress reports, and initial screening, within the electronic client record (ECR); sign documents to meet contract requirements.

  • Refer client to supportive services to supplement treatment and counseling as appropriate

  • Conduct parent-child coaching and interaction sessions

  • Maintain and carry out all scheduled appointments

  • Assist with self-sufficiently providing consultations/courtesy assistance in evaluating the appropriateness of needed interventions

  • Prepare clear, concise written reports, including treatment plans, closing summaries, and other reports as required

  • Create and maintain positive, collaborative relationships with supportive partners in the community

  • Develop a written record of appropriately used direct support funds for client family

  • Provide intervention that is family-focused, culturally relevant, behavior and goal-directed

  • Ability to travel to families' homes and communities required. A valid driver's license, car insurance, and access to a vehicle are required.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Must have respectful, open, unbiased, accepting attitude toward clients and their families and practice with cultural competency

  • Must have proven skills in creativity, problem solving, team building, resource development, mediation, etc.

  • Must be able to collaborate with community providers

  • Must be able to work in a multicultural setting

  • Requires strict adherence of HIPAA standards in the office workspace to ensure client confidentiality

EDUCATION and/or EXPERIENCE:

Minimum of a Bachelor's Degree in social work, psychology or counseling; at least two years' of experience working with high needs families within the last five years.

Experience may be substituted for education. Effective at working to support education, substance use disorder treatment, behavioral health, and other presenting concerns.

Must have respectful, open, unbiased, accepting attitude toward clients and their families.
