The Registered Nurse (RN) Case Manager is responsible for assessment and coordination of a continuum plan of care for the hospital patient populace. Performs quality and utilization review of care, making timely referrals to ensure that the patient is receiving the appropriate care, in the appropriate setting and using the appropriate utilization standards as set by community and professional standard as adopted by the medical staff.

Assures that the patients from all age groups proceed efficiently through the course of hospitalization and beyond through the continuum of care. Performs other duties as needed.

Associate's Degree in Nursing required. Bachelor's Degree in Nursing preferred.

Minimum of two years of clinical experience required. Minimum of one year of utilization review and discharge planning experience required.

Able to read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position (i.e. MS Windows, MS Office including Word, Excel and PowerPoint, internet, timekeeping, e-mail); operate and troubleshoot basic office equipment required for the position.

Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place and patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.

Active California Registered Nurse (RN) license required. Basic Life Support (BLS) certification from the American Heart Association required. Certified Case Manager (CCM) preferred.

Service Line: Home Health Rehab Job Title/Position: Quality Care Manager (QCM) Reports To: Quality Director Job Description Summary: The Quality Care Manager is responsible for reviewing patient assessments to ensure appropriate and effective care is provided to each patient in compliance with agency policies, procedures, and regulatory requirements. Trends or patterns of inaccuracy identified by the QCM will be communicated to the Quality Director to assist in the development of the performance improvement and education plan. Essential Job Functions/Responsibilities: * Reviews electronically submitted documentation and provide feedback to clinicians regarding deficiencies or OASIS corrections to ensure the medical record reflects compliance with skilled need, medical necessity, and homebound status if required * Ensures visit utilization is supported by individual patient assessment * Reviews additional orders for appropriateness * Ensures clinical documentation supports Recertification/Discharge decision * Reviews and approves final Plan of Care (485) within HCHB * Reviews OASIS for inconsistencies and locks timely * Collaborates with clinicians as necessary regarding documentation * Partners with the Quality Director and Education Director on a variety of quality initiatives * Monitors performance improvement initiatives regarding patient outcomes * Maintains working knowledge of home health regulations, conditions of participation and OASIS * Demonstrates the ability to utilize SHP data to improve quality and optimize performance with real-time actionable analytics * Attends Case Conference periodically and is available to staff for promoting professional growth and development * Partners with team members to ensure adequate coverage of workflow for each office Position Qualifications: * Must be a graduate of an accredited School of Physical Therapy * Current unencumbered license to practice as a Physical Therapist * Minimum of 1 year of home health experience with a working knowledge of Title XX11, Medicare Home Health Conditions of Participation, and OASIS * Excellent written and oral communication skills required * Ability to handle stressful situations/deadlines. * Strong computer and software skills with knowledge of Home Care Home Base a plus.
