Case Manager - Lincoln High School

SUMMARY: Works to coordinate, support and implement State and Federal IDEIA compliance procedures and expectations through a collaborative partnership with parents, students, building and district level staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Demonstrates sound judgement and emotional stability.

• Maintains a positive attitude and works well with others.

• Ensures the safety of students.

• Interacts with staff, students and the community in a positive and respectful manner.

• Consults and collaborates with other school personnel in collecting and analyzing information on a student.

• Assists staff and parents in the referral and identification of students with suspected disabilities including the completion of referral forms, gathering of background information, notification to parents and obtaining informed written parent consent for evaluation.

• Schedules and coordinates initial or re-evaluation meetings, including providing parent notification prior to the conference and any follow-up necessary.

• Schedules and coordinates IEP or other meetings, including providing parent notifications prior to or after the conference and any follow-up necessary.

• Monitors and implements adopted policies and procedures for students with disabilities including providing input to staff on form completion.

• Supports the enrollment of new students, including requesting records, reviewing documents for compliance and processing paperwork.

• Maintains systematic record keeping for all students and forms, including timely submission of required forms to the district office for electronic EMIS and compliance reporting.

• Provides on-going and effective communication with all team members.

• Other duties as assigned by supervisor.
