LHH is currently recruiting for a Case Manager in Vancouver, WA. In this role you will be assisting adults with disabilities and older persons by assessing their needs and helping them obtain in-home and community resources.
This is a direct hire opportunity. Essential Functions: Perform comprehensive functional assessments and reassessments for clients.
Develop appropriate care plans to assist clients in achieving identified outcome objectives. Implement short-term and long-term intensive service plans, including authorizing specific in-home and community services to meet client needs. Resolve client concerns regarding service delivery quality.
Coordinate with providers of client services. Plan for and follow up on service termination. Utilize in-house nursing staff for case consultation, service plan development, and provider oversight.
Maintain manual and computerized client records. Coordinate services with other community service providers, including mental health and disability service organizations. Minimum Qualifications:
Master’s Degree in behavioral or health science and one year of paid social service experience OR Bachelor’s Degree in behavioral or health sciences and two years of paid social service experience OR Bachelor’s Degree and four years of paid social service experience. Possession of a valid driver’s license, minimum state-required vehicle insurance, and reliable transportation. Knowledge of local in-home and community options and resources for the elderly and adults with disabilities and their caregivers. Ability to travel to and from clients’ homes and other community agencies, which might not be ADA accessible Compensation: $59,000-$75,000 annually
