Job Title Case Manager-Johnson City Downtown Day Center Department Office of Practice Pay Grade 6 Job Category Administrative Job Summary

The College of Nursing, Office of Practice, is accepting applications for a Case Manager. This position is located in the Johnson City Downtown Day Center.

The Case Manager reports to the Day Center Director and is responsible for providing ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual client needs; enhancing the quality of client management and satisfaction through the integration and functions of case management, utilization review, and goal planning; having accountability for the care, coordination, and goal planning of all clients; and performing other related duties as assigned.

Knowledge, Skills and Abilities

  • Knowledge of Microsoft Office and Electronic Health Records.

Required Qualifications

  • Bachelor's degree or higher in Social Work, Psychology, Sociology, Public Health, or a related field required.

Preferred Qualifications

  • One year of experience working with the homeless population

  • Two years of direct work with individuals receiving medical care or social services.

Salary $43,280
