TITLE: Case Manager Intern
STATUS: Internal (DU Grad Students) - 10-month position
COMPENSATION: Stipend will be offered TBD
DEPARTMENT: Student Success & Engagement
REPORTS TO: Associate Director Araceli Gomez
MISSION OF The TRANSTIONS PROGRAM
Dominican University's Transitions program gives students academic support as they make the transition from high school into college. The program helps prepare students for a successful first year of college and provides ongoing academic, social-emotional, and psychoeducational support to help students stay on course through graduation.
PRIMARY PURPOSE/POSITION SUMMARY
The case manager internship is a position in the Division of Student Success & Engagement. The case manager intern provides support to Dominican Undergraduates with specific focus on their social and academic integration into Dominican University. Utilizing predictive measures, this position will provide wrap around holistic support plans for the Transitions students. This position will also work directly with Transitions seminar faculty and students, providing direct psychoeducational and community building support.

JOB COMPONENTS/PRINCIPLE DUTIES AND RESPONSIBILITIES
Student Success and Engagement
Participate in weekly Student Care Team meetings.
Review academic alerts, flags, and referrals assigned to Transitions students and conduct outreach to these students.
Refer to on campus and community professional supports.
Develop psychoeducational and team building programming for Transitions students on various topics such as trauma and other mental health or social issues.
Participate in team meetings with Core Transitions Team.
Work in collaboration with Transitions seminar faculty.
Attend Transitions seminars and coordinate programming offered during seminars.
Support Transitions co-Directors in identifying student trends with development of academic self-concept and social determinants of health impeding academic success.
Utilize a strength and resiliency-based approach to working with the students and team. Using student information, offer support for the design and implementation of summer Transitions programming.
Assist with crisis intervention and referral.
Support Transitions faculty with student needs as determined/needed.
Administrative Responsibilities
Assist the professional staff in developing assessment tools.
Attend regular staff meetings.
Attend regular 1:1 meetings with supervisor.
Maintain approx. 15 office hours per week.
Other duties as requested, delegated, or assigned
QUALIFICATIONS:
Knowledge, Skills and Abilities:
Excellent written and verbal skills, organizational skills, and attention to detail.
Ability to work autonomously and as a member of a team.
Ability to work in a strength based culturally responsive manner.
Must be able to work some nights and weekends.
Proficiency in SSE Office programs.
Interest in school social work, higher education, and/or work with underrepresented groups.
Bilingual in Spanish (preferred).
Education and Experience:
Bachelor's degree
Enrollment in a Dominican University MSW program is required.
