The Case Manager

  • Coverage role is a rewarding opportunity to directly make an impact on individuals' lives and provide essential support from the get-go. This role typically does not carry an active caseload. It supports with the Intake and Enrollment processes which involves being the first point of contact for assistance, addressing inquiries, conducting assessments, and coordinating resources as needed. The Case Manager

  • Coverage role requires empathy, communication skills, and a strong commitment to helping others navigate through challenges and access the support they need.

Developmental Pathways (DP) has an opportunity for a Case Manager

  • Coverage in our Systems Navigation department who will manage caseloads in coverage to ensure support is provided, aligning with rules and regulations. This role will also complete coverage tasks for individuals in coverage including required documentation related to screening and referral, eligibility determination and assessments, support plan development, PMPM activities, monitorings, and other tasks as needed. In addition, this role will provide support with coverage management, research, and assignments and will work with Program Manager to determine needs amongst coverage. This role will act as a systems navigator, assessing an individual's overall needs and assisting with exploring programs, long-term services and supports, and other community resources to meet needs.

This is a 40-hour week, hybrid position, working remotely and coming into our headquarters office in Aurora, CO and/or off-site locations, as needed. This is a Monday

  • Thursday or Tuesday

  • Friday schedule.

Requirements

  • Bachelor's Degree; or

  • Five (5) years of experience in the field of Long-Term Services and Supports; or

  • (LTSS), which includes Developmental Disabilities; or

  • Some combination of education and relevant experience appropriate to the requirements of the position

  • Valid Colorado motor vehicle license and provide proof of motor vehicle insurance

  • Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years

Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Preferred

  • One (1) year of experience in developmental disabilities/long-term services and supports

Additional Responsibilities

  • Serve as subject matter expert for applicable programs and services; serve as resource/point of contact for complex cases

  • Complete various assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools

  • Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies

  • Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meets all required timelines

  • Complete required documentation related to monitoring activities; evaluate need and effectiveness of services requested and delivered

  • Schedule, facilitate, document, and complete follow-up for all required and ad-hoc meetings; disseminate updates to the necessary parties

  • Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties, including, but not limited to providers, individuals, and family members

  • Maintain knowledge of state/ federal regulations, policies and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs and other community resources

  • Maintain a professional and ethical manner in all interactions and meets performance quality, customer service, and coordination standards as assigned by department leadership; maintain appropriate boundaries
