MAJOR FUNCTION:

Facilitates a patient's hospitalization from pre-admission through discharge. Collaborates with physicians, nurses, social workers and other members of the healthcare continuum to ensure appropriate, cost-effective care.

Applies clinical expertise and medical necessity criteria to perform the functions of utilization management, care coordination, resource utilization, and discharge planning. Must utilize established criteria to review appropriateness of admission and continued stay. Perform initial and ongoing assessment to ensure a timely transitional care plan is developed that will support each patient's needs and prevent hospital readmission.

Identify and prevent potential barriers in transition of care. Ensure discharge plan is consistent with patient's clinical course, continuing care needs, and covered services. Identify patients and/or families who require social work referral.

Actively participate in multidisciplinary rounds. Collaborate with next level of care providers as appropriate.

QUALIFICATIONS:

Education:

BSN required if hired after September 1, 2021. Hired prior to BSN preferred, Associates Degree required.

Experience:

Minimum 3 years of experience in hospital setting required. 1-3 years of case management experience preferred; clinical background in

assigned unit/service line preferred; previous utilization management experience preferred; working knowledge of Milliman criteria

preferred; previous experience with regulatory and compliance requirements preferred. Demonstrates excellent communication skills

(oral and written) and interpersonal skills necessary to communicate with families, patients, interdisciplinary healthcare team members,

vendors, and third-party payors for optimum care. Excellent clinical and administrative skills and judgement. Proficiency in Microsoft

Office. Ability to work independently, setting priorities to coordinate care plans efficiently.

Certification/Licensure:

Licensed Registered Nurse in New Jersey required, and Case Management Certification preferred.

This position is responsible for implementing change, serving as a coach and mentor, and being the subject matter expert for performance improvement methodologies. This position will have a lead role in strategic design with Executive Leadership to provide insights and recommendations to improve operational efficiencies across the health system. <br><br> The Manager of Operations Consulting Group will actively facilitate meetings and work sessions with executives, develop data-driven recommendations, apply change management skills to instill ownership, and establish sustainment plans to ensure permanent adoption and success of improvement initiatives. In addition, this role will be responsible for leading and collaborating with Senior, Managing, and Associate Operations Consultants within the department, as well as staff across all levels of the organization, to proliferate LEAN Six Sigma and change management practices.</p><p>QUALIFICATIONS:</p><p>A. Education: Bachelor's degree required. <br><br> Master's degree preferred. <br><br> Major in Healthcare, Business, Industrial/Systems Engineering, or related field preferred.</p><p>B. Experience: Minimum 6 years of relevant experience preferred. <br><br> Managerial or other leadership positions preferred. Expert knowledge and experience with continuous performance and process improvement initiatives, including project and change management, required. <br><br> Must possess advanced knowledge utilizing Microsoft Excel, PowerPoint, Visio, and Word. <br><br> Extensive experience interpreting data trends, patterns, and gaps to inform performance improvement priorities. Experience with data visualization and translating data to be understood across all levels of the organization required</p><p>Essential to possess self-starting qualities with the ability to work independently while providing guidance to others. Must have prior experience planning and managing performance improvement projects, as a leader or supporting team member. <br><br> Expert written and verbal communication skills, including presentations, required. The Manager, Operations Consulting must have an expert ability to work with and influence staff across all levels of the organization to supporting lasting improvements.</p><p>C.
