Natchaug Hospital is Eastern Connecticut's leading provider for children, adolescents and adults with mental illness and substance abuse addictions. We offer a variety of programs including inpatient and outpatient behavioral health, residential treatment for court-involved teenage girls, geriatric services, clinical day treatment schools and chemical dependency services. Job Summary

The Case Manager is responsible for providing a range of services for clients with either Psychiatric and/or Substance Abuse issues. Also provides advocacy, outreach, and service linkage for clients and initiates access to entitlement services.

• Works collaboratively with the Therapist and other members of the multidisciplinary treatment team to coordinate aftercare arrangements for the client.

• Facilitates groups as required

• Ensures personal, client and unit/staff safety.

Qualifications

• Experience working with a behavioral health and/or chemical dependency population

• Bachelor's degree in Human Services or related field, Social Work preferred

• Valid driver's license may be required based on program

• Knowledge of community resources preferred.

• Computer proficiency and keyboarding skills required
