In collaboration with the mental health department and health providers at Venice Family Clinic (VFC), the case manager position will advocate for patients and help them address social, emotional and personal challenges to improve overall wellbeing. Supervised by a clinical social worker, the case manager will help connect and follow up with patients who need services outside of VFC (e.g., testing, housing, benefits, higher level of care, school-based services and evaluations, etc.). Work will also include attending meetings at schools, in the community and within the clinic.
Job Qualifications:Required:
Skills in customer service and follow through
Excellent bilingual (Spanish/English) communication skills
Knowledge of various community resources
Experience in working with low-income population and the homeless
Ability to establish and maintain cooperative working relationships with staff, parents and representatives from local agencies
Valid California driver's license or California ID
Working knowledge of word processing programs
Comfort and acceptance in working with marginalized populations including LGBT, teens, the disabled, mentally ill, religious minorities and with all options of pregnancy outcomes
