Description
We are seeking a Case Manager II to work on our Waiver Case Management team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. This is a full-time hybrid position in St. Louis County in the Virginia & Hibbing area. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.
Hours: Full-Time, Monday- Friday, business hours 8am-4pm
Location: St. Louis County, Minnesota | Hybrid position Must have physical presence in St. Louis County
Hiring Range: $21.60 - $23.50/ Hourly, based on qualifications $1,000 Hiring bonus to external candidates

Main Job Duties:
Must have physical presence in St. Louis County.
Flexibility to work with case loads in the south and north areas.
Attending bi-annual meetings with clients to monitor services.
Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers.
Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed.
Making referrals and authorizing services based on needs.
Maintaining records and documenting all coordination activities.
Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.

Passing all background and requirements for facility and position; and
