Description
Case Manager II
Our Deaf and Hard of Hearing Services team is seeking a Case Manager II to join our Targeted Case Management team to work with clients who are diagnosed with chronic mental illness. This position is a full-time, hybrid opportunity that will support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.
American Sign Language (ASL) is the primary mode of communication which offers our residents a strong sense of community with other deaf adults and strengthens our support activities in a culturally affirmative environment. We provide an array of individualized services to our residents to help maintain independent living in a community setting.
Hours: Full-Time, Monday- Friday, business hours 8am-4pm
Locations: Twin Cities, Minnesota counties | Hybrid Physical presence is required
Hiring Ranges: $23.00 - $26.00/ Hourly, based on qualifications
Hiring Bonus: $1,000 to external candidates

Main Job Duties:
Responsible for running day to day duties at the drop-in center working with deaf participants.
Coordinating and facilitating community-based outings, engaging clients with events and activities.
Using person-centered principles, actively engage with clients in their daily activities, and develop and maintain appropriate relationships that support the client in meeting their individual goals.
Document interactions with clients into organization's Electronic Health Record (EHR) and if applicable, program-specific documentation/record-keeping system, while providing clear and concise communication with client, staff and other applicable parties.
Conducting outreach activities and sourcing to build engagement with potential clients.
Requirements
Requirements for All Case Manager II positions:
Education and Experience Required for Targeted Case Manager II position:
Bachelor's Degree in behavioral science or related field
2,000 hours of supervised experience working with adults with severe and persistent mental illness
Fluent in American Sign Language
OR
Master's Degree in behavioral science or related field.
Fluent in American Sign Language
Preferred Qualifications for Targeted Case Manager II position:
Experience providing direct services to adults with serious and persistent mental illness
Experience with case management and commitment proceedings
Knowledge of Recovery, Motivational Interviewing and Person-Centered approach
