Job Overview

The Case Manager II position will be responsible for providing comprehensive case management and outreach services to HIV positive clients. The Case Manager II will work as part of an interdisciplinary team in a primary medical care facility for persons living with HIV/AIDS. The CM II will follow the program's Scope of Work activities to achieve the program goals through interviewing, counseling, assessment, caseload organization, mentoring and management. This position will also provide work oversight and mentoring to other members of the department.

Minimum Requirements:

1.High School Diploma required.

2.Bachelor's degree in Social Services, Sociology, Human Services, Counseling or related field required.

3.Minimum of 5 years of experience working with HIV client and under-served communities required.

4.Knowledge of community resources for public assistance programs.

Salary Range

$32.57 - $40.71 hourly
