Come join an incredible team! MaineGeneral Health, the third largest health care system in Maine, is seeking an experienced Case Manager I to join our team!
The Case Manager I is responsible for managing complex patient issues in relationship to needed services and the patient's transition through the healthcare system. As a patient advocate, this person works collaboratively with patients and families and with the inter-disciplinary team both internal and external to the organization. The ideal candidate can work independently with effective time management and problem solving skills.
Formulates and implements a plan that addresses the patient's identified needs and issues by assessing the patient/family needs, issues, resources and care goals; determining the choices available to individual patients; and educating the patient/family on the choices available.
Identifies and effectively utilizes community resources to meet the needs of patients/families.
Assists the patient to identify and/or develop their support system and encourages the patient to utilize their support system.
Collaborates with physicians, other healthcare team members, the patient's payer, and health system administrators in order to optimize clinical outcomes.
May participate in ongoing evaluation of the patient's plan of care through the Provider or Licensed Clinical Social Worker's (LCSW) review, a bimonthly interdisciplinary team review and/or peer supervision.
Documents patient phone calls and assessments in the scheduling and/or EMR system.
The ideal candidate will have
Three years or more of experience preferred.
Maine State Board of Social Work Licensed Master Social Worker (LMSW) or Maine State Board of Social Work Licenses Clinical Social Worker (LCSW) or Maine State Board of Social Work Licensed Social Worker (LSW).
Certified Case Manager (CCM) preferred.
Utilization Review experience preferred.
