Case Manager I

Performs technical and administrative work providing assessment, case management and linkage to federal, state and other programs administered in the Department of Community Services for indigent residents and/or seniors. Develops case plans with clients; provides comprehensive case management and supportive counseling; coordinates outside agency referrals for services; determines eligibility for assistance programs; provides follow up services; conducts home visits if required and maintains case documentation. Work is performed under general guidance of a supervisor who reviews work through observation of the employee's skill in performing the necessary casework functions, review of casework and analysis of reports indicating results and outcomes.

QUALIFICATIONS:

Bachelor's Degree in Social Work, Sociology, Psychology or related field; minimum of one (1) year of experience in social work. Equivalencies: Unrelated Bachelor's Degree and two (2) years of related experience; related Associate's Degree and three (3) years of related experience; unrelated Associate's Degree and four (4) years of related experience; graduation from high school or an equivalent recognized certification and five (5) years of related experience.

PREFERENCE FOR EXPERIENCE: Providing self-sufficiency and homeless prevention/services; conducting home visits; facilitating oral presentations/public speaking.

Also desirable: Related Master's Degree; bilingual (English/Spanish/Creole); knowledge of interviewing skills/medical terminology/case note documentation styles (must specify on application).

Valid Florida Driver's License and PBC Risk Management Department driving history approval prior to appointment.
