We are looking for dedicated, caring and compassionate people to work in our Residential/Outpatient Treatment Facilities and Community Programs.

Case Manager (Halfway House)

Schedule per week: Full Time (Monday

  • Friday 8:30am

  • 4:30pm)

Job Summary:

The Case Manager functions as an integral part of the treatment team and provides primary social service care to a caseload of residential addiction treatment clients in residence.

Responsibilities:

  • Provide case management for a caseload of residential addiction treatment female clients with children in residence, including the following tasks:

  • Serve as a liaison between Straight & Narrow, Drug Court/Probation and all other social service agencies, including the scheduling of Case Conferences as necessary.

  • Ascertain whether client needs birth certificates, social security cards or an identification card. Determine status of their TANF, food stamps, Medicaid benefits (if applicable).

  • Send for ad/or make appointments for client to go for any of the above that are missing.

  • Open clients welfare case in Passaic County. Obtain any documents necessary to do this. Close clients welfare case in sending county (if applicable).

  • Obtain prior authorization for clients receiving Medically Assisted Treatment (MAT)

  • Open clients Medicaid case immediately upon receipt of all required documents (if applicable).

  • Run Medicaid reports on all active clients monthly and renew clients Medicaid if applicable

  • Make contact with clients relatives to bring items needed and/or go to the store and purchase these items.

  • Go over clients shopping lists monthly. Remove any inappropriate items. Check clients current supplies to see that they are not over-ordering. When client returns from store, compare what is on the list with what he has bought.

  • Educate/Assist client with regards to money management and budgeting strategies.

  • Schedule weekly meetings with clients and provide them with phone calls to their probation officers, etc.

  • Attend any Case Conferences that are scheduled.

  • Schedule outside appointments for welfare, WIC, social security, Probation visits, Section 8 applications, job search, trips to IOP for interviews, etc.

  • Escort clients on shopping trips, to court appearances, on interviews related to Section 8 housing, job interviews and at all times when they are looking for an apartment.

  • Apply for Section 8 housing for the client, obtaining all necessary documents in order to complete her application.

  • When the client secures an apartment, apply for Emergency Assistance. Make arrangements to have clients lights, gas and phone turned on. Make sure deposits are made for clients rent and utilities.

  • When Section 8 housing is not available, work with referral source to find alternative housing.

  • Arrange an appointment for client to be interviewed and complete necessary paperwork at IOP before client is discharged. Coordinate with IOP to arrange for a medical appointment so that there is no gap in medical services if client is taking medication. Identify a child care provider in a timely manner and make application for the clients child/children.

  • Close out clients account at S & N before he leaves, transferring all personal documents to him. Fill out form that gives client a checklist of what to do upon discharge and give her a letter to take to her first welfare appointment.

  • Close out clients Passaic County welfare case (if applicable).

  • Facilitate case management group and didactics as scheduled by supervisor

  • Conduct Case management assessment of client within the first month of treatment

  • Submit documentation of all groups, didactics and individual client sessions to supervisor daily.

  • Maintain social services portion of clients charts as per the Division of Mental Health and Addiction Services (DMHAS) and CARF guidelines, ensuring that the file is up- to-date for all documentation.

  • Attend regularly scheduled MDT meetings for clients on caseload, providing information on each clients progress, legal situation, social service benefits and housing situation.

  • Provide referrals for clients to other agencies (i.e., health, psychiatric, vocational rehabilitation and public assistance) for concurrent and continued social services and care.

  • Compile referral forms from counselors for Psychiatric services and submit referral paperwork to co-occurring liaison

  • Upon completion of APN evaluation, forward all prescription to Pharmacy within 24 hrs of client being seen

  • As applicable, create a Medical Administration Record (MAR) for all clients who are on prescribed medication.

  • Monitor all MARS for accuracy and compliance daily

  • Log in all clients medication on to the MARS forms as needed

  • Encourage clients attendance and participation in AA/NA/GA

  • Provide crisis intervention as necessary.

  • Follow all Electronic Health Records (EHR) policies.

  • If Case Manager does not have either a CADC or LCADC, must obtain certification within three (3) years from date of hire. Must be actively pursuing and provide documentation quarterly, of progress towards obtaining certification.

Qualifications:

Must have a BA or MA degree. Enrollment in CADC/LCADC courses, experience in case management and knowledge of addiction and co-occurring preferred. Must be computer literate and possess ability to type and have a valid Drivers license.
