Position Overview

Under the direction of the Clinical Director, the Case Manager provides non-clinical services to assist individuals in accessing necessary medical, social, educational, and other resources. The primary purpose of Case Management is to support an individual's rehabilitation from a diagnosed disorder by organizing needed services and supports into an integrated system of care until the individual can assume responsibility for these services.

Full-time or part-time positions.

Essential Duties and Responsibilities

  • Facilitates access to available community resources based on client needs to maximize social integration.
  • Assess client or family needs and eligibility for services.
  • Works collaboratively with clients, families, and appropriate community agencies to coordinate and facilitate services.
  • Evaluates, facilitates, and monitors services and client progress toward established goals and objectives; documents promptly and accurately in the Electronic Health Record (EHR).
  • Monitors treatment plans as directed; provides follow-up and discusses areas of opportunity with the Clinical Director and Primary Therapist.
  • Co-facilitates group sessions upon approval from the Clinical Director and with appropriate training or education.
  • Demonstrates a strong understanding of and ability to engage in core Case Management activities, including linkage, monitoring and follow-up, referral, advocacy, and crisis intervention.
  • Effectively plans and organizes work, demonstrating flexibility in scheduling to meet the needs of clients and HCBH programs.
  • Consults and collaborates with the Clinical Director to resolve scheduling conflicts.

Candidate Qualifications and Experience

  • Bachelor's Degree in Human Services field of study.
  • Prefer minimum 3 years' experience in medical and Behavioral Health.
  • High degree of organizational skills and attention to detail.
  • Successful completion of HCBH pre-employment screening and background check.
  • Has the ability to communicate effectively orally and in writing.
  • Has a valid driver's license and eligible for HCBH insurance coverage(s).
  • Proficient in Microsoft Office: Word, PowerPoint, Excel, Outlook.
  • Ability to conceptualize, develop and implement programs to their completion in a fast-paced environment.

While performing the duties of this job, the employee is required to walk, stand, sit, and use the hands. Occasionally stoop, crouch, or kneel. The employee must occasionally exert or lift up to 25 pounds.
