Case Manager

Avisa Recovery is seeking a compassionate and dedicated Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive case management services to clients in our care.

Your primary responsibilities will include:

• Assessing clients' needs and developing individualized care plans
• Monitoring clients' progress and adjusting care plans as needed
• Collaborating with interdisciplinary team members to coordinate and integrate services
• Assisting clients in accessing and utilizing community resources
• Providing education and support to clients and their families
• Maintaining accurate and up-to-date documentation

We are looking for an individual who is organized, detail-oriented, and has excellent communication and interpersonal skills. Experience working in a similar role and familiarity with mental health and substance use disorders is preferred.

• Strong communication and interpersonal skills
• Ability to work effectively as part of a team
