This position performs and manages all aspects of case management, conducting telephonic case management for complex, high utilization members to include identification and assessment of needs, planning and coordination of care, and monitoring outcomes in accordance with GlobalHealth and the departments policies and procedures. Responsibilities include: determining the appropriateness of services, medical necessity, and location required to encourage effective, high quality care and cost efficient outcomes. This position requires being proactive when it comes to identifying high priority members and coordinating care to improve the long-term well-being of our members. Excellent communication skills and the ability to develop a rapport with members, in order to best assess their needs, is required. Identifying barriers to care and conducting follow ups with members to ensure the highest quality of care are also primary duties of the position.
