Responsibilities

SPECIALIZED TREATMENT FOR OPTIMIZED PROGRAMMING (STOP) services are designed to assist recently incarcerated offenders in their reintegration to the community. Specifically, STOP 2 located in Campbell, CA, under contract with the California Department of Corrections and Rehabilitation (CDCR) provides housing and in/outpatient program services to individuals under the supervision of the CDCR Division of Adult Paroles (DAPO). STOP 2 as a contract manager identifies and contracts with community-based providers. STOP 2 staff facilitate the placement of individuals into appropriate community services; monitor participants progress in programs; develop and monitor participant discharge plans; monitor community-based providers for adherence to contract requirements and evidence-based practices. STOP 2 staff facilitate ongoing audits of providers and participants via on site visits and physical inspections, as well as monitoring both participants and providers with the CDCR provided data base (ARMs)

Summary

This position is the secondary provider of casework services to the participants. The primary focus of this position is discharge planning, and interfacing with Paroles staff. The Case Manager to participant caseload ratio is designed at 1 to 50.

Primary Duties and Responsibilities

  • Provides interactive services to the STOP participants.

  • Conducts placement assessments with participants

  • Works with and communicates regularly with Community Based Providers

  • Works/Communicates with Providers and Paroles staff as part of the overall development of Case Management Plans (CMP) with participants, however this staff persons primary focus is Discharge Planning.

  • Monitors the participant's progress relative to their Case Management Plan through all phases of the program.

  • Makes appropriate referrals to outside agencies as necessary.

  • Maintains progress notes in the ARMs electronic database.

  • Keeps the assigned Parole Agent) apprised of participant's progress.

  • Develops discharge plans.

  • Conducts monthly progress and discharge planning sessions with each assigned participant to both review and update their discharge plan and readiness..

  • Ensures implementation of the specific program treatment/intervention modality under the direction and supervision of the Clinical Supervisor.

  • Manages and coordinates individual participant services through appropriate referrals and CBP program assignment.

  • Collaborates with participant to identify a network that will support the participant's reintegration into the community.

  • Assists, as directed, the Compliance Administrator and Clinical Supervisor in conducting and following up on community-based provider audits.

  • Participates in all training provided for STOP staff; prepares schedules, reports, or other information required for the program and requested by the STOP Program Manager, Clinical Supervisor, Compliance Coordinator and/or CDCR personnel.

  • Attends GEO/STOP-professional development, supervision, training, staff meetings as required.

  • Performs other duties as assigned.

Qualifications

Minimum Requirements

  • High school diploma or its equivalent and a minimum of one (1) year experience working with criminal justice populations,

  • OR a bachelor's degree from an accredited university.

  • A valid driver's license is required.

  • Ability to recognize the importance of family, social networks, and community systems in the treatment and recovery process.

  • Knowledge and the ability to comply with program documentation requirements, applicable ethical and professional standards of conduct.

  • Ability to facilitate the participant's engagement in the treatment and recovery process.

  • Possession of the case management skills necessary to facilitate discharge planning, consult as a member of a multi-disciplinary team and assess ongoing treatment planning efficacy.

  • Ability to work with computers and the necessary software typically used by the department.

Preferred Skills and Experience:

  • Possession of an Alcohol and Drug Counselor Certification recognized by the Department of Health Care Services (DHCS) preferred.

  • Understanding of a variety of models including cognitive behavioral therapy, and criminogenic risk factors

  • Ability to conduct assessments and/or evaluations, which serve as the basis for CBP placement.

  • Ability to provide services appropriate to the personal and cultural identity and language of the participant

  • Ability to appropriately use strategies from accepted and culturally appropriate models with participants with substance use and other criminogenic risk factors.

  • Ability to establish rapport with participants including management of crisis and determination of need for additional professional assistance.
