Crossroads Programs, Inc., an Apis Services, Inc. a New Jersey affiliate, is seeking a dedicated and passionate Case Manager to join our team of trained professionals in Mt. Holly, NJ. About Mount Holly House:

Our Mount Holly House offers a nurturing space for up to five females aged 14 to 17. Our dedicated team provides 24/7 care, creating a comfortable living environment where young women can thrive and develop. Through a holistic approach, we offer a range of services to encourage growth and empowerment. Our team has a proven track record of building confidence, fostering self-expression, and imparting new skills to our residents.

Qualifications:

  • Bachelor's degree in Social Work, Psychology, or related human services field.

  • Minimum of three years of experience working with at-risk youth.

  • Must be 21 years old.

  • A valid driver's license with a clean standing over the past 6 months is required.

  • Physical & Drug Screening, Criminal Record History, FBI, and Child Abuse History Clearances.

  • All new hires are required to successfully complete Safety Crisis Management (SCM) certification within one week of hire.

  • Training Overview:

  • Duration: The SCM program spans three days.

  • Physical Demands: Active participation in physically demanding activities is necessary.

Preferred Qualifications:

  • Experience in case management within a behavioral health setting.

  • Proven experience working closely with a clinical/multidisciplinary team.

  • Excellent organizational and time management skills.

  • Highly effective communication and documentation skills.

  • Proficient in engagement and rapport-building with youth.

  • Versed in trauma-informed care models, Nurtured Heart Approach, and Six Core Strategies.

Summary of Essential Duties:

As a Case Manager, you will play a crucial role in advancing ethical practice and professionalism, ensuring the protection of emotional stability, and fostering healthy childhood development. Your responsibilities include:

  • Compile Information:

  • Gather all relevant information and documentation related to youth.

  • Service Coordination:

  • Participate in service coordination on behalf of youth and families under the supervision of the MDT and/or licensed therapist.

  • Collaboration with MDT:

  • Collaborate with licensed therapists, youth, family/guardians, and MDT members to provide care coordination input in the development of individual services plans, treatment plans, and IMDS tools.

  • Liaison Role:

  • Serve as a liaison for school, community, and ancillary services.

  • Representative Role:

  • Represent the youth and family on the agency's behalf in screening, placement, court appearances, and other relevant situations.

  • Consistent MDT Contact:

  • Remain in consistent contact with all members of the youth's MDT, ensuring seamless communication and collaboration.

  • Youth-Centered Advocacy:

  • Provide youth-centered advocacy, championing the needs and rights of the youth and their families.

  • Referral and Linkage:

  • Provide referral and linkage to ancillary and specialized treatment services and interventions to address specific needs.

  • Site Visits:

  • Routinely conduct school, home, and ancillary visits to gain a comprehensive understanding of the youth's environment.

  • Special Appointments:

  • Schedule and attend special appointments, facilitating necessary services and support for the youth.

  • Correspondence Management:

  • Take responsibility for correspondence on behalf of the youth and family, including treatment team meeting invitations, as well as monthly/quarterly plan dissemination to MDT members.

  • File Management:

  • Ensure effective file management, maintaining accurate and up-to-date records of youth and family information.

  • Participation in Meetings:

  • Actively participate in treatment conferences, individual educational plans, child family team meetings, emergency review meetings, MDT meetings, Rounds, Clinical Rounds, and Record Reviews.

  • PerformCare Authorization:

  • Take responsibility for PerformCare authorization status, ensuring Medicaid coverage eligibility for youth under your care.
