Education and Experience:
Bachelors Degree in Behavioral Health, Psychology, Health Behavior Science, or related field
Experience working with adults with severe and persistent mental illness and/or substance use disorders
CPR and First Aid Certification
Valid drivers license and permission for a review of driving record to meet insurance requirements
Responsibilities:
Professionally participate in team conferencing to create and implement care plans for clients
Maintain a therapeutic alliance with clients
Refer and link clients to all needed services provided by an outside agency
Coordinate independent living assistant, medical and vocational services
Work with community agencies to develop needed resources including housing, employment options, and income assistance
Provide support and consultation with the clients family
Provide in-home services including ADL training, medication assistance and training, social support, and home management assistance; providing supportive counseling
Document and keep other staff, clinical coordinators, and Program Director informed of all changes in the consumers mental or social status
Carry primary or secondary cell phone as assigned and responds to after-hour and emergency calls in accordance with program directives
Provide education and work as an advocate for persons served in the community
