Provide case management services that empower youth, young adults, and families to participate in a community environment. Serving families with a child (through the age of 18) who has been diagnosed as having Serious Emotional Disturbance (SED) or Developmentally Disabled and young adults (18-26) with a Serious and Persistent Mental Illness (SPMI). Services are individualized and efforts are focused on educating and empowering consumers so they may achieve their goals, direct their own lives, increase their natural supports and resources and are less dependent on service providers. Case Management seeks to help individual/families better understand problems and partners with individuals/families so they may work on creating alternative solutions, providing coordination and monitoring of service delivery to assure the consumer has access to appropriate treatment and services.

Qualifications:

Minimum, BSW (LBSW); MSW/MA (LLMSW, LMSW, LLPC, LPC, TLLP, LLP) preferred. One year of related experience preferred.

Must be trained in CAFAS or ability to be trained as a reliable rater of CAFAS. Valid Michigan Driver License.

Partnering with families for the well-being of children is our top priority. As a Case Manager you will provide casework and supportive services to children in foster care, foster families and biological families referred to the Refugee Services program. * Meet weekly with supervisor to consult on cases, to review case plan and direction and to evaluate client participation and progress. * Keep abreast of community resources and refer families for support as needed. * Maintain effective communication with community agencies or individuals involved in service provision. * Attend training provided internally and in the community and apply newly gained knowledge in providing services. * Be available as co-therapist as appropriate. * Be available to translate both written and verbal communication for program clientele and staff. * Assist in the study of and licensure of foster families. * Arrange for at least monthly meetings with foster parents and foster children. * Prepare for and be in attendance at required court hearings. * Provide transportation for clients as needed. * Maintain accurate, up-to-date information on foster children in care while meeting DHS, ORR, and Bethany's documentation and reporting requirements. * Assist in the training of new staff. * Facilitate and assist in providing on-going education and support services for foster parents. * Attend education and support meetings provided to foster parents. * Lead or co-lead a therapeutic or educational group. * Perform other duties as assigned. Qualifications: * BSW or BA/BS in a human service field. * Bilingual. * Demonstrate excellent clinical, therapeutic and crisis intervention skills. * Demonstrated verbal and written communications skills. * Eligible to obtain professional licensure or registration. * Computer skills sufficient to perform essential functions. * Maintain a reliable automobile with adequate insurance coverage. * Knowledge of state, community and agency resources. * Prior work experience with refugee or minority families and demonstrates cross-cultural sensitivity. * Subscription to and integration of the agency Statement of Faith, Mission Statement, and Cultural Diversity Commitment.
