Job Summary: This position reports directly to the Director of Nursing and/or the Administrator for the Extended Care Unit (ECU). The Case Manager (CM) collaborates with nursing staff, physicians, therapists, outside agencies, and other members of the healthcare team to coordinate care for Extended Care residents and coordinates a plan for a safe discharge; provides consultation and case review as appropriate; participates in hospital committees and community and professional organizations as needed; acts as a liaison between the ECU team and various hospital departments; performs related duties as required.# Primary duties are to assist with discharge planning, helping identify needs for residents and family as they arise during the resident#s stay for appropriate disposition.
They may arrange for at-home services, such as home health care and issue Durable Medical Equipment (DME) as appropriate. This individual will promote continuity of patient care with physicians and long term facilities to ensure that resident needs are addressed in a timely manner. This individual supports the achievement of optimal resident outcomes and unit operations by promoting individual growth, team functioning, and interdepartmental cooperation within the framework of shared decision-making and coordinated care.
This individual will manage and maintain all insurance approvals, authorizations, and appeals on the resident#s behalf as appropriate.# This individual will be responsible for verifying insurance coverage of the resident and provide resources needed for payment arrangements as needed.# This individual must demonstrate the Witham Health Services philosophy of caring in all aspects of job performance and interpersonal relationships. # # Minimum Requirements:# Bachelors degree required in a healthcare#field.# Will consider RN, Social Worker, Therapist or Health and Human Service degrees.
(CCM) and/or a Social Service Designee#Certification preferred. Minimum 4 years previous experience working in a Case Manager involved with discharge planning.
