Position Summary
Under supervision, provide case management services (in a licensed or unlicensed program) which includes case record keeping, advocacy, and service coordination for consumers (adults with varying degrees of learning, behavioral, emotional, and developmental disabilities) in the program.
Essential Duties and Responsibilities
Develop, prepare, and maintain complete case records, notes, and other paperwork for all referrals and consumers according to Licensing, Regional Center, DDS, and ECF standards. This includes case notes, Consumer Individual Risk Assessment forms, evidence of negative TB tests, physicals, etc.In consultation with program staff, develop a plan of individual service for each consumer that is designed to promote greater independence. Establish consumer goals and objectives initially, quarterly, semi annually and annually. Monitor consumer progress and work closely with staff on the implementation of goals and evaluation of each assigned consumers progress. Participate in the intake and assessment process of new consumers. Assist in the development and implementation of behavior management programs and provide crisis intervention to consumers as needed. Monitor consumer attendance.Participate in team meetings, safety committee meetings, and in-service training. Assist in providing in-service training to other direct service staff.Establish and maintain good working relationships with consumers, parents and/or care providers, and referral/service provider agencies. Conduct annual IHC/IPP meetings with consumers, parents and/or care providers and regional center representatives and other meetings as necessary.Adhere to ECFs Injury & Illness Prevention and Safety Program. Report work related injuries/illnesses that involve participants/consumers. Implement ECF guidelines as they relate to medical emergencies, suspected abuse or neglect and unusual incidents. Assist facility staff during emergency medical situations including implementing First Aid, CPR, NCI, and transporting of consumers to home or medical facility if necessary.Keep abreast of community resources and advocate on assigned consumers behalf for emergency (i.e., medical, psychiatric, substance abuse, etc.) and support services (i.e., speech therapy, social/recreational services, independent living skills training, etc.). Be familiar with all available ECF programs and services. Provide in-service training to consumers regarding safety, self-advocacy, etc.
Other Duties
On an as-needed basis, serve as instructor for consumers in day programming, paid work, and volunteer placements in facility and community-based training.Comply with ECFs Injury and Illness Prevention Program. Ensure safe use and operation of tools and/or equipment.Comply with ECF policies and procedures, including mandated reporting requirements.Regular, reliable attendance.Perform other duties as assigned.
Qualifications
Education/Licenses/Certifications: BA in Human Service Field
Experience: Two years working with individuals who have special needs in a direct client service capacity or one year as a Case Manager in a social service environment.
Knowledge/Abilities/Skills:
Excellent verbal and written communication skills and basic math skills.Ability to work on multiple projects simultaneously.Computer literate (MS Word and Excel).Detail oriented with a high degree of accuracy.
Requirements
Valid California drivers license, reliable transportation, evidence of car insurance, and insurable driving record.Pass a DOJ background investigation.Provide evidence of a negative TB test initially and every three years thereafter.Pass a pre-employment physical (licensed programs only).Obtain and maintain NCI, First Aid, CPR and Medication Administration certification.Bilingual, preferably in SpanishOccasional weekends working studio sales or vendor fairs; up to 3 events a year for Case Managers to engage in.

Ability to implement NCI, First Aid, and CPR.Assist non-ambulatory consumers with transferring to/from wheelchairs (pushing, pulling) lifting with assistance.Ability to lift/carry items that weigh up to 20 pounds.Frequent sitting, moving from one area to another, speaking, and listening.Reach with hands and arms and use hands and fingers to handle objects, tools, operate copy machine, fax machine, printer, computers, and/or other devices.Ability to read printed materials and a computer screen.Ability to communicate in person and over the telephone.
Work Environment
This is a program for adults who have varying degrees of learning, behavioral, emotional, and developmental disabilities. Employee may encounter consumers who are in crisis, exhibiting emotional/psychological distress or behaving in an aggressive manner. Work will be performed primarily indoors in a shared office. Environment may include frequent interruptions, loud noise, odors, varying indoor temperature, and drafts. Outdoor temperature and weather will vary.
