JOB SUMMARY: This position is responsible for conducting the comprehensive educational and intensive case management services for pregnant and parenting teens participating in the Future Promises Program.

This position requires the provision of appropriate assessment, intervention and evaluation of the teens and their children through quality and consistent execution of the program goals and objectives. QUALIFICATIONS : Bachelor’s Degree in Nursing, Public Health, Social Work, Health Education/Administration Minimum of three to five years experience in working with minority at-risk children, adolescents and their families in a school, community and home setting. Preferred maternal/child health background Experience in public speaking Experience in community-based health promotion programs Valid driver’s license and vehicle insurance KNOWLEDGE, SKILLS, & ABILITIES:

Excellent written and verbal communication skills and a keen ability to pay close attention to detail Excellent time management and organizational skills with the ability to be multi-tasked and function effectively under stress and deadlines Knowledgeable of normal child and adolescent growth and development; positive relationship building skills; pregnancy, fetal development and the process of childbirth; adolescent reproductive health including adolescent sexuality development, contraception, and STI/HIV/AIDS; high school graduation requirements, Title IX, and college entrance procedures.
