GENERAL DESCRIPTION OF RESPONSIBILITIES:
In this role, the Case Manager will be responsible for utilizing an integrated case management model that includes assessment, planning, implementation, care coordination, and monitoring.

MINIMUM QUALIFICATIONS:

Bachelors degree in a Behavioral Health related field preferred
Minimum 5 years experience in Health Care industry.
Knowledge of the Case Management and Utilization review process
Must possess a valid Arizona drivers license, insurance coverage, reliable transportation, and a clean driving record
Proficient computer & keyboarding
Basic data analysis knowledge
Great interpersonal skills
Excellent communication skills (written & verbal)
Strong attention to detail
Highly organized
Must be able to work in a diverse, multi-cultural environment and be sensitive to the service populations cultural and socioeconomic characteristics

CASE MANAGER 2 Division of Developmental Disabilities 333 N. 8th Avenue Safford, AZ 85546 www.azdes.gov JOB SUMMARY: The Case manager 2 serves members with developmental disabilities, e.g., Autism, Cerebral Palsy, Intellectual Disability and Epilepsy by supporting them and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities. JOB DUTIES: * Research community for available resources to meet needs identified in Individual Support Plan (ISP) and to inform individual of resource options. * Providing support coordination that consist of identifying needs and available services and resources, planning, obtaining, and monitoring the effectiveness of contracted medical and non-medical services in meeting our members' identified needs * Facilitate meetings, program visits and attend meetings to assist with individual/family choices. * Complete referrals for assessed/covered services. * Develop, review, monitor and evaluate Individual Support Plan (ISP) in accordance with mandated timelines. * Approve type and level of assessed services within approved guidelines. * Collaborate with community agencies to provide assistance and support to individuals and families in crisis. * Write comprehensive progress notes and maintain case records. * Meeting regularly to assess needs and supports, monitor progress, or make changes to the ISP, which includes travelling at different times throughout the day within and outside business hours to conduct onsite-visits to member's home, placement, schools or program sites; this activity represents 30% of work time and may require driving your own personal car (Mileage will be reimbursed) KNOWLEDGE, SKILLS AND ABILITIES (KSAs): Knowledge of: * Methods, procedures, and techniques used in the development of Individual Support Plan (ISP) * Health Insurance Portability and Accountability Act (HIPAA) * Principle and practices of case management * Community resources * Medical and behavioral health issues and terminology Skill in: * Oral and written communication * Interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public * The use of various software (Microsoft Word, Excel, etc.) * Interviewing and interacting with members and family members * Problem identification and resolution Ability to: * Work independently and within diverse work teams * Shift priorities and adjust to changes * Organize and manage case load * Maintain confidentiality * Drive a motor vehicle SELECTIVE PREFERENCE(S): * The candidate must possess a degree in Social Work; OR 2 years of case management experience serving persons with physical or developmental disabilities. * Bilingual (English and Spanish) is preferred PRE-EMPLOYMENT REQUIREMENTS: * Have a current, valid state issued driver's license appropriate to the assignment. Employee is subject to driver's license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12) * Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees * Successfully pass background and reference checks.
