Personal Service Coordinator, Part-Time

21 hours per week / $24.00 per hour

(The actual compensation will be determined based on experience and other factors permitted by law.)

Summary: Under the supervision of Program Manager the Personal Service Coordinator (PSC) is required to provide support to the daily operation of the Opportunity Center which provides access to basic services as well as Harm reduction, harm reduction safety classes, sex safety tools, information and referrals, short-term case management for supportive services and case management to homelessness individuals in Opportunity Center who are experiencing homeless, substance use challenges, and engaging in sex work by utilizing motivational interviewing, harm reduction practice modalities. Provides linkages to employment, housing, crisis housing, Mobil clinic, case management, document support, that supports members barriers and needs.

Essential Duties and Responsibilities: This is not an exhaustive task list, and other duties may be assigned deemed appropriate or necessary by program manager.   • The PSC is required to complete intakes & track members entering into the Opportunity Center with Logs and worksheets provided

  • Provides supply distribution and trainings and safety courses and trainings

  • Provides members with supportive safe, non-judgmental environment

  • The PSC will assist and support members with any barriers as well as link them to the supportive services such as employment, case management, medical & insurance, documentation support assistance, mobile clinic, health care and any supportive services they may need.

  • Ensure members are linked and supported with accessing medical and mental health care, and safe substance use services as well as overdoes prevention education and services.

  • Provides individualized coaching and positive reinforcement to members in the areas of budgeting skills, hygiene, meal preparation, public transportation, and other independent living skills necessary for housing success.

  • Ensure that all member interactions are person-centered, goal-focused, and in line with evidence-based practices such as trauma informed, harm reduction tools/supplies trainings, motivational interviewing, and housing first model.

  • Provides mediation and advocates on behalf of members to best support their needs

  • Must be knowledgeable of community resources and provide linkage as needed to resources within the community and support members in navigating the process.

  • Provide ongoing collaboration and communication with Full-Service Partnership (FSP) teams.

  • Communicates effectively in oral and written forms of communication with members.

  • Must attend and participate in CES Case Conferencing, all-staff, team meetings, community meetings, and other meetings as assigned.

  • Maintains proper documentation required for program compliance and accountability in accordance with MHALA

  • Attend ongoing training as directed for professional growth and to effectively meet the needs of members.

  • Responsible for reporting abuse, as a mandated reporter which includes immediately reporting any concerns concerning violation of a persons rights, actual and/or suspected abuse

  • Attends mandatory trainings and meetings

Other Position Requirements:

  • High school diploma or GED, plus two years experience working in social services field and or schooling

  • Demonstrate knowledge of community resources and show compassion and empathy to our homeless neighbors

  • Must function well in a team-oriented approach to member services.

  • Knowledgeable in computer applications such as Microsoft Word, Excel, and Outlook as well as possess organizational skills

  • Driving with current class C drivers license preferred not required

  • Be able to work in office Monday through Friday schedule 8 am to 4 pm

  • TB clearance and re-testing every year are conditions of continued employment

  • COVID vaccination is required for this position. Accommodations will be considered upon proof of a legal exemption.
