As a child case manager at West Central Behavioral Health you can change a child's life West Central is the nonprofit community mental health care provider for Sullivan County and the Upper Valley, ensuring access to advanced counseling, treatment, and support for people of all ages, regardless of their ability to pay.
By joining our established team of dedicated professionals at our Lebanon, NH office, you will be helping children and their families living with mental illness, by providing services in the community and office-based treatment through case management, skills training, psycho-education and professional support focused on assisting consumers in working toward their practices throughout the Sullivan County.

Responsibilities:
Provide case management, IROS, and IMR services to assigned case load, both in the office and community sites in keeping with agency and program missions so that their community tenure and quality of life is improved.
Assist clients in managing daily problems and stresses, developing a new life structure via work or other meaningful activities, addressing and developing life and illness management skills, and providing family education and support.
Coordination of services with other agencies, application and management of benefits, developing and implementing a treatment plan reflecting an outcome orientated and function based philosophy.
Work as a team member with other case managers in the Child Program to ensure that comprehensive and quality care is provided to clients at all times.
Maintain all documentation that is required in a complete and timely manner.
Provide all clinical services in ways that are outcome orientated, recovery based philosophy that is both effective and efficient.
Attend and participate in staff supervision/training and attend clinical case presentation to assist in treatment planning and problem solving.

Requirements:
Bachelors Degree (will consider near-graduates and those with an AA degree in an appropriate filed and related work experience)
Excellent driving record
Reliable and insured vehicle
Valid drivers license
Case management experience helpful
The knowledge of social work, psychology and human services

Additionally, to assist customers with their health care needs by filling prescriptions and providing excellent customer service. The incumbent is also required to perform all tasks in a safe manner consistent with corporate policies and state and federal laws. The associate is responsible for the functions below, in addition to other duties as assigned: * Ensure the accuracy and appropriateness of all prescriptions filled by completing Drug Utilization Review and Final Quality Assurance, applicable to state and federal Board of Pharmacy regulations.• Build profitable business and script growth through recommended clinical programs including appropriate immunization and when available MTM (Medication Therapy Management), DCS (Diabetes Care Specialist), and all ongoing other programs as identified.• Oversee the daily activities of the Pharmacy department.• Counsel customers with regard to medications filled at the pharmacy.• Ensure growth and profitability of Pharmacy Department.• Handle and resolve customer issues, complaints and questions to build customer trust and loyalty.• Comply with all federal and state laws and regulations.• Interact with physicians to gain additional information about customers and prescriptions to be filled.• Ensure excellent customer service by Pharmacy associates.• Maintain a clean and efficient Pharmacy department.• Provide leadership and development for associates by communicating career opportunities, providing regular performance feedback, and demonstrating RAPTAR(Recognition, Appreciation, Praise, Treat Associates Respectfully) behaviors.• Maintain appropriate security of the Pharmacy department.• Maintain reports for controlled, outdated, and recalled medications; and prescription and customer files.• Manage pharmacy inventory at acceptable levels to fulfill customer need.• Ensure compliance with all policies and procedures for controlled substance dispensing and record keeping.• Prioritize Pharmacy Department tasks and follow through to ensure all tasks are completed in a timely manner.
