POSITION SUMMARY

The Cathedral Green and Francis Xavier Plaza Family Specialist will provide direct Case Management services to the Service Enriched families as well as non-service enriched families as needed. This individual will maintain a case load of a minimum of 28 Families. Services provided consist of but not limited to helping residents with entitlement applications, referrals for basic needs, addressing the barriers to lease compliance while encouraging personal growth through Employment and Educational Workshops. The individual will also promote self sufficiency keeping appropriate boundaries with Residents and collaterals. The preferred individual will have sensitivity to Cultural differences.

GENERAL DUTIES/ RESPONSIBILITIES:

  • Work closely with residents to develop Case Management Service plan.

  • Review the Service Plan with residents three months after the initial Service Plan update to ensure that measurable goals are met by a 6month time line. Update, edit or create new Service Plans when residents experience life altering events.

  • Meet with Residents at a minimum of once bi-weekly ensuring that the focus of the meeting is in coordination of the residents service plan.

  • Life Skills Teaching which includes but is not limited to cooking, housekeeping, personal care, initiating help for services, coordination of personal and medical appointments, time management, etiquette and parenting.

  • Obtain(s) release of information with all collaterals.

  • Assist residents with applying for and maintaining entitlements. (Annual Re-certifications & Re-determinations)

  • Initiate/Facilitate Employment Preparedness, Educational, housing stability and financial literacy workshops/fairs.

  • Referrals to services for Low income families, elderly tenants, persons with disabilities, veterans and emergency assistance.

  • Maintain a resource directory of all providers.

  • Work collaboratively with Property Management and Clinical Services to ensure the residents are supported holistically while maintaining the appropriate boundaries.

  • PQI Functions

  • Ensure the consistent accurate collection of the data

  • Identify patterns and trends for program

  • Use results of data to inform supervisor of trends that may impact services

  • Adheres to all Agency Policy and Procedures outlined in the Policy & Procedure Manual, while carrying out the Mission and Vision of the Agency.

QUALIFICATIONS:

  • Associates Degree, preferably in a human service field or High School/GED with knowledge of Community Resources.

  • Minimum of 2 years experience in the Social Services/Human Services field.

  • Strong Assessment Skills with the ability to identify a crisis situation.

  • CPR Certification/Computer Literacy.

  • Experience working with Homeless, Veteran, Mental Health & Substance abuse population.

  • Five-Six years of sustained recovery required.

  • Proven ability to submit timely, detailed case notes.
