Position Title: Case Manager

Essential Functions: The Case Manager utilizes professional nursing skills to assess patient and family scenarios for clinical appropriateness for the setting and establishes plans for effective management of the continuum of care. Assessment, planning, intervention and reassessment are performed in a collaborative environment. The key goals of the Case Manager include proactive, individualized planning for patients' progress that optimizes quality of care, utilization, cost and patient satisfaction.


Educational Requirements / Qualifications:
  • Bachelor's Degree Preferred

  • Diploma/Associates Degree in Nursing

  • Professional Certification in Case Management, preferred

Licensure/Certification/Registration

  • Florida Registered Nurse License

Special Skills/Qualifications/Additional Training/Experience Required

  • Effective communication skills to facilitate contact with physicians, patients/clients, families and staff

  • Minimum of 5 years experience, Case Management experience preferred.

  • CCM certification preferred

  • Must be able to read, write, speak and understand English.

VIEW JOBS 1/21/2021 12:00:00 AM 2021-04-21T00:00 JOB SUMMARY: Our Adoptions Case Manager Supervisor plays a tremendous role in overseeing Adoptions Case Managers that help children achieve permanency in a forever home through their continued efforts within the Adoptions Unit. Moving a case along from Dependency to Adoptions, their focus is to maintain quality communication with our pre-adoptive and prospective adoptive parents, and recruit families which helps to alleviate the consumers concerns when dealing with an often lengthy and frustrating system. Additionally, our Adoptions Case Manager Supervisor ensures that Adoptions Case Managers make appropriate, necessary, and accurate referrals for services that are provided timely to achieve the highest level of care for our children, as well as move towards finalization of the adoption in as timely a manner as possible. FUNCTION: Ensures continuity of care while facilitating and expediting Adoption permanency for children through a Case management model that develops, expands, accesses and links resources in the community that meet the needs of children and their pre-adoptive families. RESPONSIBILITIES: * Serves as the Adoptions Case Management Unit Supervisor providing Case management oversight, team supervision and guidance. * Oversees and tracks critical data elements involved with management reports via the FSFN database. * Oversees and performs all Adoptions Recruitment involved tasks and responsibilities. * Performs ongoing analyses of contractual performance indicators to ensure unit compliance. * Completes case file audits to ensure appropriate and timely referrals to community service providers in accordance with established case plans. * Oversees provision of Adoption services, including planning, linking, facilitating match and disclosure staffings, monitoring, and advocacy, to assigned children within assigned case of the unit. * Monitors and manages all unit reports and outcomes, including monthly visitation, judicial progress, finalizations and case status, among others. * Participates in staff development programs and opportunities. * Participates actively in team meetings, client care and utilization reviews, staff meetings, Adoption Transfer Staffings, Permanency Staffing, AARC Reviews, PRIDE Meetings, and all other professional forums that promote safety, well-being and expeditious finalizations, as appropriate for children served. * Provides administrative and management duties related to supervision of staff, including interviewing and selection, training and orientation, supervision, performance management and development and scheduling. * Establishes and maintains effective working relationships required for proper functioning within the role of Adoptions Case Manager Supervisor, including: * Relationships as a member of the interdisciplinary team within the unit, particularly around backing up other team members as appropriate and expected; * Relationships with supervisory staff, emphasizing proper response to and use of supervision and appropriate reliance on supervisory staff to guide their activities during unusual circumstances; * Relationships with community resource providers, particularly emphasizing facilitation of the referral process and providing continuity of care; * Relationships with children and their Caregivers, pre-adoptive/adoptive families including awareness of and attention to developmental issues related to interpersonal functioning within the family environment. * The ability to comply with the regulations requiring a workplace of dignity and respect and equal employment opportunity compliance. * The ability to work collaboratively with others and to accept constructive criticism from supervisors and managers. * Willingness to abide by the companys published rules and regulations COMPETENCIES: Knowledge, Skills, Abilities, and Motivation (KSAM) required for an employee to be successful. * Serving Our Consumers: Effectively meeting consumer needs, building productive consumer relationships, and taking responsibility for consumer satisfaction and loyalty. * Consumer Focus: Building strong consumer relationships and delivering consumer-centric solutions. * Values Differences: Recognizing the value that different perspectives and cultures bring to an organization. * Effective Communication: Clearly conveying information and ideas to individuals and groups in a manner that engages the audience and helps them understand and retain the message. * Communicates Effectively: Developing and delivering multi-mode communications that convey a clear understanding of the unique needs of different audiences. * Manages Conflict: Handling conflict situations effectively, with a minimum of noise. * Continuous Improvement: The ongoing process of achieving positive changes in service and processes to improve efficiency and quality. * Resourcefulness: Securing and deploying resources effectively and efficiently. * Cultivates Innovation: Creating new and better ways for the organization to be successful. * Expertise: Demonstrating proficiency of an employee's skill or knowledge within their field of practice. * Drives Results: Consistently achieving results, even under tough circumstances. EDUCATION & EXPERIENCE: * Required: Bachelors degree in Human Services or related degree with at least 3 years experience in directly working with children and families in the child welfare system of Case, with one year of experience providing supervision to direct service workers or related field preferred. Possess current Child Welfare Certification through the Florida Certification Board. * Preferred: Masters degree in psychology, counseling, social work or Human services/social services related degree with at least 3 years experience working Adoptions cases in the child welfare system of Case. TRAINING & DEVELOPMENT: Completes all requirements according to Individual Training Plan within the first six months of hire and annually. Other training and/or professional development may be assigned due to evolution of programs. Ensures completion of State of Florida Child Welfare Case Management Certification trainings as required by the Florida Certification Board. Completes agency-specific training within the first six months of employment. Possesses a current Background Clearance Screening Letter as required by DCF. Possesses a valid Florida drivers license, have a personal vehicle to conduct direct field activities, have a valid Florida insurance card, and is insurable by the agencys current insurance carrier.
