Summary
The primary responsibility of the inpatient Case Manager is to provide coordination of care during treatment, case management and discharge planning services to the patient and family in collaboration with the clinical team in an inpatient setting. Discharge planning includes coordinating the communication and referrals to post discharge providers including home health, residential facilities, transportation, outpatient care or services, and any other type of post discharge provider of services required. The Case Manager also accepts alternate assignments, as required. This job requires a critical thinker able to work independently while interacting with patients and their families and the clinical team. This job requires superior interpersonal and organizational skills in a dynamic, fast paced environment.
Key Functions
Attend inpatient unit and team meetings
Meet with patients and communicate with families to assess for case management and discharge planning needs and implement the plan with direction from the treatment team
Facilitate communication and information exchange between outpatient providers/referents, aftercare programs/providers and the inpatient team
Coordinate care during the patient's hospitalization
Connect patient and family with other Menninger departments, as needed
Provide referrals and information to patients and families and execute the aftercare plan with direction from the treatment team to ensure a smooth discharge
EDUCATION: Bachelor's degree required. Bachelor's in social work, psychology or a related health field preferred (psychology, social work, sociology, anthropology, human services, etc.)
Experience: Behavioral health or medical case management experience preferred.
