WestCoast Children's Clinic, located in Oakland, California, is a non-profit community psychology clinic that provides mental health services to Bay Area children, youth and families. The Outpatient (OCS) Case Manager works within dynamic teams of youth, caregivers, mental health providers, intensive care coordinators, social workers, teachers and other supportive figures in a youth's life. Services may include intensive skill building, advocacy, linking to and coordinating with tutoring & vocational programs, mentoring, shelter/ housing, medical and mental health services, legal aid, education, access to benefits or other needs families may have. An OCS Case Manager provides a diversity of care through conducting the following combination of services:

  • Clinical Case Management
  • Short term & long term intensive linkage and follow-up with community resources which aim to improve mental health symptoms.
  • Individual Rehabilitation or Intensive Home Based Services (IHBS) - Focuses on skill building to improve life functioning; examples include: guidance on applying to jobs/schools, building self care and hygiene skills, strengthening social, coping and other behavioral skills.

  • Enhanced Care Management (ECM)

  • A benefit that connects youth and families to healthcare and other services.
  • Provide stabilization care to foster youth in a milieu environment as youth await and prepare for transition to foster placements.

An ideal candidate demonstrates flexibility, initiative, creativity, inclusivity and knowledge of the unique challenges youth and young adults face in foster care. Writing progress notes for each meeting and completing other clinical documentation in a timely manner are an essential part of this position - a writing sample exercise will be conducted during an interview to identify your baseline for progress note writing.

About the Program: Outpatient Services

Our Outpatient Mental Health Services are uniquely suited to meet the needs of vulnerable and high-risk children, adolescents and families - particularly those involved in the Alameda County foster care system. We see clients at our Fruitvale clinic, via tele-health as well as in community settings, meeting families at their greatest point of need, equipping them with the tools and linkages to help overcome what often are multiple traumatic events. The impact of our work is evident in the lives we help to change on a daily basis.

  • Manage a caseload and ensure 60% (24 hrs / week) are dedicated to providing services to youth and families; Uphold ethical and legal responsibilities, complete high quality documentation, link youth to to resources, support skill building, assess needs, strengths & provide crisis intervention and de-escalation

  • Enthusiastically able to work collaboratively as part of a multidisciplinary treatment team to support youth and families with developing and making progress toward their goals.

  • Openness to providing services in a variety of community settings such as homes, schools, parks, shelters, other agencies, etc.

  • Understands the value of building healthy relationships to help youth navigate complex systems and how trauma can impact youth and team dynamics; Open to Restorative Justice Practices.
  • Commitment to Ongoing Learning
  • Ability to use supervision effectively; Ability to be self-reflective and accountable to growth areas; passionate about learning and participating in ongoing training provided by WCC.
  • Progress Notes and Clinical Documentation
  • Knowledge of or ability to learn Medi-Cal reimbursement procedures and billing; This position requires the completion of daily progress notes and other clinical documentation which complies with policies and procedures for meeting EPSDT Medi-Cal and other billing requirements as set by WCC.

Qualifications:

  • Bilingual in Spanish and English (spoken & written) required.

  • 2 years of experience working with youth and families and/or providing case management, rehabilitation services or intensive home based services required.

  • Bachelor's degree in Psychology, Criminal Justice, Social Work or related field preferred.

  • Knowledge of Alameda County resources for children and families preferred.

  • Passionate about working with foster youth, families & young adults who have complex traumatic histories.

  • Highly motivated to work from a team based approach.

  • Values humility, compassion, self-care, and ongoing personal growth.

  • Flexible schedule with ability to work evenings and occasional weekends as needed.

  • Ability to travel by car to meet with clients, generally within a 90 mile radius of the WCC office. Clean CA DMV record, current car insurance and a vehicle; travel is required.

Position details and compensation:

This is a full time, non-exempt (hourly) position reporting to the Director of Care Coordination. Starting rate is 34.38 per hour ($71,500 per year), inclusive of 10% Spanish differential. Includes performance-based compensation plan, competitive benefits package, and 403(b) plan.
