ORGANIZATIONAL OVERVIEW:

Esperanza Center, a program of Catholic Charities of Baltimore, is a comprehensive and bilingual resource center whose mission is to welcome immigrants by offering hope, compassionate services, and the power to improve their lives. Since 1963, immigrants from all over the world have trusted the Esperanza Center to help them navigate life in a new country. Our dedicated staff and volunteers provide services related to education, healthcare, immigration legal matters, family reunification for unaccompanied immigrant children, and community support to thousands of immigrants each year.

JOB DESCRIPTION:

  • Bilingual in English and Spanish

Case Manager provides intensive case management including, but not limited to, as-needed home visitation services to immigrant participants / clients. The Case Manager assesses household needs, coordinates resources based on goals, and provides advocacy, information, and referral services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Develop Individualized Service Plans (ISP) with participants with the primary goal of helping families become self-sufficient within a specific 5me frame.
  • Advocate for families and individuals to access needed services in support of household dynamics, safety, education, health, mental health, legal/immigration proceedings, substance abuse, and child enrichment activities.
  • Complete and document all required reports, case notes, and contacts in the program database by the expected timeframes and requirements.
  • Collaborate with community organizations, law enforcement agencies, or government offices to ensure the safety and well-being of children.
  • Use Motivational Interviewing, a strength based coaching model, to help clients overcome obstacles to reach desired outcomes.
  • Maintain an environment that aligns with Trauma Informed Care (TIC).
  • Collaborate and participate as part of a team of professional case management colleagues dedicated to the empowerment and accompaniment of the immigrant community.

REQUIRED QUALIFICATIONS:

  • Bilingual in English and Spanish
  • Ability to utilize computer systems and software necessary to perform position functions required. Knowledge of case management software, desired.

SALARY:

  • Bachelor’s degree in related field and 2 years of case management or comparable experience (salary starts at $49,750/year) or
  • Master’s degree in related field and 5 years of related experience (salary starts at $55,120/year)
