Duties

Family Supportive Housing is committed to creating, designing, and implementing services in the community from a proactive, holistic, person centered approach. We have several full-time opening in the following areas: permanent supportive housing, Eviction Prevention, and domestic violence serving previously homeless individuals/families, and those at imminent risk of homelessness.

Provide culturally responsive, trauma informed case management services including but not limited to: intake, assessment, creating and monitoring of service plans, documenting client progress, facilitation of virtual and/or in-person visits to individuals and families in their homes, schools, community, court house, and working with other agencies and partners in providing efficient links and referrals to community resources and coordination of services in order to remove barriers to maintain and or stabilize housing (FHPAP/HUD). Ability to complete daily documentation, interpret reports/assessments from other providers, maintain complete, accurate, and up-to date client records to effectively document case activity and support case decisions. In addition, Housing Case Managers may provide specialized supports specific to their role which may include: identification of market rate housing, administering housing subsidies, performing housing inspections, and facilitation and coordination of client education groups.

Perform other duties as assigned.

This is an in-office position with potential to move to hybrid after demonstration of competency and completion of training.

Job Qualifications

BA degree in Human Service field and minimum of one year related work experience required. Strongly related work experience of at least 4 years may be exchanged for the BA requirement.

Candidates with a strong understanding of homelessness, mental health, chemical dependency, navigating housing resources (eviction prevention), domestic violence and community resources preferred. Must have valid MN driver's license, proof of insurance and meet other requirements of Wilder Foundation's driver policy.

Valid driver's license, proof of insurance and reliable vehicle required.

Ability to work independently and collaboratively.

Strong organizational skills.

Knowledge of supportive housing and how to provide supportive housing resources.

Ability to build and develop lasting relationships to provide high-level customer services

Ability to provide competent ethical services, crisis intervention, and referral services to clients of diverse cultural, ethnic, racial and socioeconomic backgrounds.

Utilize harm reduction interventions and methodologies to provide client centered services that keep them in housing.

Knowledge of social service programs and an ability to advocate for service.

Ability to analyze complex situations and evaluate potential for improvement.

Ability to organize and prioritize a high-volume workload efficiently.

Ability to seek out resources and information.

Demonstrated ability to communicate effectively, positively, and professionally with participants, providers, family members, and others.

Preferred

Training in working with low-income families and those who are homeless preferred.

Social Work license preferred.

Knowledge of related issues in chemical dependency, mental health, parenting, sex trafficking, and domestic violence preferred.
