Responsibilities

Job Summary

A Case Manager manages treatment services of assigned caseload including initial and ongoing coordination of care, program engagement, advocacy, behavior intervention, LOC transitions, aftercare, follow-up and discharge planning. Case Manager communicates with the Counseling and UR teams on all aspects of patient care. American Addiction Centers strives to ensure that all patients receive the most comprehensive care to address issues that may impede their recovery.

Duties and Responsibilities: .

  • Perform all case management requirements in accordance with designated scope, state laws, substance use treatment boards, and professional ethical guidelines.

  • Maintain a patient caseload.

  • Provide and document weekly coordination of care designed to improve patient adherence to treatment and follow-up care. This includes contacting providers and collecting medical records upon admission into treatment as well as overseeing patient connection and appointments with post-treatment providers and services.

  • Complete aftercare assessment, discharge plans, and required contacts according to policy timelines.

  • Maintain and document communication with Business Development rep. including the provision of ongoing professional coordination with the referent, family, legal, monitoring programs, EAP, and/or employer as necessary.

  • Facilitate life skills and recovery groups as assigned.

  • Establish patient connection and use of alumni apps.

  • Provide and document weekly sessions with patients that address recovery stability, adhering to prescribed treatment, active coordination of care, transition of care issues, behavior intervention, coordination of care, and discharge planning.

  • Manage, address, and document patient behavioral concerns and crisis situations as needed.

  • Attend weekly treatment team meetings as assigned per caseload.

  • Address aftercare issues related to ongoing treatment needs, MAT, transitional housing, employment, transportation, support groups, community resources, healthcare appointments, and other support services.

  • Complete and/or manage completion and timely transmission of necessary treatment documentation for FMLA, Short-Term Disability Insurance, ROI's, and other outside agency reports as needed.

  • Ensure that all documentation meets policy, licensure, accreditation, and standards for services in a timely manner following the AAC's documentation policy guidelines.

  • Communicate efficiently with other departments to ensure the best care for patients.

  • Understand and abide by State Department standards, facility accreditation, as well as AAC policy and procedures.

  • Identify, evaluate, and address situations which may be potentially harmful to patients and report any patient or safety concerns to the assigned counselor and Clinical Director.

  • Adhere to company code of ethic and maintain professional boundaries with patients at all times.

  • Act in a manner that is respectful of patient rights and demonstrates an understanding of each patient's unique recovery needs.

  • Communicate with all staff, patients, and families using excellent professional written and verbal skills.

  • Participate in required clinical supervision, training, and staff meetings.

  • Work as a larger interdisciplinary team along with other departments to ensure quality of care.

  • Demonstrate dependability and adaptability with patients and staff.

  • Maintain confidentiality of patient and company information.

  • Abide by the standards of HIPAA and 42 CFR Part 2 guidelines.

  • Maintain upkeep of personal professional certification and CEU's as required by state boards without lapse, holds, suspensions, or investigation.

  • Assists in monitoring and managing patient community as a therapeutic milieu.

  • Other tasks as assigned by direct supervisor within scope of practice.

Qualifications

Education/Experience:

  • Bachelor's Degree in Behavioral Sciences such as Human Services, Social Work, Psychology or other related field required.

  • State recognized (IC&RC) alcohol and drug counseling certification (or intern SUD counselor certification) required.

  • Minimum of 2 years experience in a behavioral healthcare field and/or substance abuse recovery field.

  • Experience in behavior health treatment including, but not limited to, group facilitation, behavior intervention, community liaison, as well as case management for individuals diagnosed with a substance use and/or mental health disorder.

  • Knowledge of utilizing electronic health record software and documentation.

  • Knowledge of ASAM criteria.

  • Ability to read, write and interpret written information, type documentation, present information to groups and participate in meetings.

  • Working knowledge of Microsoft Word, Excel, and Outlook.
